Treatment Of Primary Dysmenorrhea With Tongjing Fang And The Effects Of Prostacyclin And Thromboxane

Objective:By observing the clinical effect of Tongjing Fang for primary dysmenorrhea(PD)of Syndrome of Spleen and Kidney Asthenia,and to explore the clinical efficacy including the improvement of dysmenorrhea and the change of symptom scores.To investigate the therapeutic mechanism of Tongjing Fang for PD by detecting the levels of plasma TXB2 and 6-keto-PGF1?.Method:Sixty patients with primary dysmenorrhea were randomly divided into treatment group(30 cases)and control group(30 cases).The patients were divided into treatment group(30 cases)and control group(30 cases)from January 20 to January 2017 in Jiangsu province.The Treatment group:Two days before menstruation and end of menstruation,Tongjing Fang,200ml,oral,1/day,sooner or later sub-service;non-menstrual,Jianpi Bushen Decoction,200ml,oral,1/day,morning and evening service.Control group:Two days before menstruation and end of menstruation,Dysmenorrhea Treasure Granules,1 bag/time,2 times/day,warm boiled water;non-menstrual treatment,ditto.3 menstrual cycle for a course of treatment,observe a course of treatment.The changes of dysmenorrhea,the changes of symptom score and the contents of plasma TXB2 and 6-keto-PGFla were observed before and after treatment.SPSS21.0 software was used for data processing and statistical analysis.Results:1.Dysmenorrhea:The treatment group was cured in 13 cases,markedly effective in 10 cases,effective in 5 cases,ineffective in 2 cases,the total effective rate was 93.3%;The control group was cured in 5 cases,markedly effective in 7 cases,effective in 11 cases,ineffective in 7 cases,the total effective rate was 76.7%,the treatment group was better than the control group(P<0.05).2.TCM symptoms efficacy:In the treatment group,10 cases were cured,12 cases were effective,6 cases were effective,2 case was ineffective,the total effective rate was 93.3%;the control group was cured in 4 cases,markedly effective in 7 cases,effective in 11 cases,8 cases,the total effective rate was 73.3%The treatment group was superior to the control group(P<0.05).3.Improve the symptoms:the treatment group was better than the control group P<0.05,which in improving the menstrual flow,aversion to cold,waist and knee ache in the treatment group was significantly better than the control group.4.Comprehensive effect:Treatment group recovered in 3 cases,markedly effective in 10 cases,effective in 15 cases,Ineffective in 2 cases,the total effective rate was 93.3%.5.Compare TXB2 values:the treatment group was significantly lower than before treatment(P<0.05).There was no significant difference between the two groups before and after treatment(P>0.05).The TXB2 value of the two groups was significantly lower than that of the control group(P<0.05).6.Compare 6-keto-PGFla values:The treatment group was significantly lower than before treatment(P<0.05).There was no significant difference between the two groups before and after treatment(P>0.05).The TXB2 value of the two groups was significantly lower than that of the control group(P<0.05).7.CompareTXB2/6-keto-PGF1? values:The treatment group was significantly lower than the control group(P<0.05).8.Safety indicators:By observing the blood,liver and kidney function(ALT,AST,BUN,Scr),ECG were not found abnormal..Conclusion:The clinical effect of Tongjing Fang for primary dysmenorrhea is not only to alleviate the symptoms of dysmenorrhea,but also to improve the symptoms of the whole body.The clinical effect is 93.3%,and Tongjing Fang is safe and non-toxic side effects.The mechanism of dysmenorrhea may be related to the increased level of plasma 6-keto-PGF1?,the reduced level of TXB2 and TXB2/6-keto-PGF1? ratio,and the improved uterine micro vascular contraction status.
