OBJECTIVE:Utilizing cone beam computed tomography(CBCT)and related software to process condylar in skeletal class ? adult patients with mandibular deviation,measuring the required measurement items,analysing statistical data,discussing and researching positon and morphological characteristics and the correlation with mandibular deviation,providing references for clinical diagnosis and design.METHODS: Twenty skeletal class ? adult patients with mandibular deviation were selected as the deviated group and twenty adults with normal occlusion were selected as the control group.Both of them came to visit the department of orthodontics in Shenyang stomatology hospital during the period from October 2014 to October 2016.CBCT scans of their temporomandibular joints were taken under principle of informed consent.Invivo Dental 5 was used to reconstruct the temporomandibular joint of patients,process the condyle and measure required items,that is,condylar anteroposterior diameter,condylar medialateral diameter,condylar maximum area,length of anterior slope,length of posterior slope,condylar apex height,condylar perpendicular height,inner joint space,outer joint space,medial joint space,Anterior joint space,posterior joint space,superior joint space,length of interior point,length of outermost point,radius,condylar intersection angle.A series of measurements of thecondylar morphology and position were performed.The results were entered into Microsoft Excel,then compiled into SPSS 22.0.The collected data were analyzed using paired t-test.The difference between the two groups was compared,and the test standard was statistically significant when P<0.05.RESULTS:(1)Condylar morphology in patients with mandibular deviated group,the condylar maximum area(98.42±17.59),condylar apex height(2.91±1.41)and condylar perpendicular height(20.96±3.58)of the deviated group are smaller than the undeviated group and the condylar anteroposterior diameter,condylar medialateral diameter,length of anterior slope,length of posterior slope are little difference.(2)Condylar position in patients with mandibular deviation group,the outer joint space(1.77±0.53),Anterior joint space(3.01±0.96),superior joint space(2.76±1.15),posterior joint space(2.49±0.76),inner joint space(2.45±0.8)between the deviated group and the undeviated group are difference.(3)There are little differences between the two sides of condylar in normal occlusion.(4)Comparing the condylar morphology and position with two groups:,the results showed that there are smaller medial joint space(3.24±0.61)and bigger inner joint space(3.22±1.07),length of outermost point(59.17±3.12),radius(51.46±2.21)in the deviated group.CONCLUSION:(1)The two sides of condylar morphology that skeletal class ?adult patients with mandibular deviation are obviously asymmetric,the deviated group is smaller than the other.(2)The two sides of condylar position that skeletal class ? adult patients with mandibular deviation are obviously asymmetric,the deviated group moved upper,outer,more backward,and the forward,downward,inward displacement of the the other group.(3)Patients with Skeletal Class? high angle of condylar height is higher than normal occlusion.(4)There are little differences between the two sides of condylar in normal occlusion.(5)Because of the complex structure of the temporomandibular joint,the method of cone beam CT can be used to provide strong data support for the clinic,which is helpful to the correct judgment of the orthodontic doctors.
