BackgroudIn recent years,a large number of studies have shown that there is a lower risk of tumor progression and more sensitive to radiation and chemotherapy treatments in oral cancer patients who human papilloma virus(HPV)are positive,as well as the prognosis of HPV positive patients is much better in HPV positive oral cancer patients when compared with patients with HPV negative oral cancer.These suggest that HPV plays a very important role in the development of oral cancer.But at present,the literature reports about HPV infection in oral cancer differ greatly,and there is not much discussion about the etiology between the oral cancer and HPV infection,and its specific pathogenesis mechanism is still unclear yet.In adition,some studies reported that the expression of p16 protein has close correlation with HPV infection,considering p16 IHC staining as a surrogate marker of HPV infection.Northern Anhui is one of the hotspots of oral cancer,and what's the rate of HPV infection in oral neoplasms in this region? And what's the level of p16 protein expression in oral neoplasms in this region? What's the correlation of HPV infection and p16 protein? These still have not been reported,and remain to be clarified.ObjectiveTo investigate the infection rate of HPV and p16 protein expression in oral neoplasms in Northern Anhui and their significance.MethodsForty four cases of oral neoplasms fresh tissue specimens were collected from the first affiliated hospital of bengbu medical college at the time from October 2014 to October 2015.Age and gender of the patient,smoking,drinking and other basic cases were recorded.Meanwhile,20 cases oral nonneoplastic tissue of patients(including oral trauma or snoring orthopedic surgery,inflammation and cyst)in the hospital were collected as a control group.HPV DNA was measured by PCR to identify the infection of HPV.The expression of p16 protein was evaluated by immunohistochemistry.Mutual correlation between the HPV and p16,and the meaning of the both for oral cancer were analyzed,respectively.ResultsThe infection rate of HPV in oral neoplasms and nonneoplastic tissue was 47.7%(21/44)and 10%(2/20),respectively.The positive rate of p16 in oral neoplasms and nonneoplastic tissue was 77.3%(34/44)and 0(0/20),respectively.No correlation between p16 protein expression and HPV infection in oral neoplasms(P>0.05).In oral tumor tissues,there was a higher proportion in male,non-smokers and non-drinkers in the HPV positive group compared with the negative group,though there wasn't a statistically difference(P>0.05).Moreover,there was a higher proportion in male,non-smokers and non-drinkers in p16 positive group when compared with p16 negative group.However,there wasn't a statistically difference(P>0.05).ConclusionHPV infection is closely associated with oral cancer in Northern Anhui.The positive rate of p16 protein expression is higher in oral cancer patients and no correlation between p16 protein expression and HPV infection in oral neoplasms.IHC staining of p16 cannot be used as a surrogate marker of HPV infection.It is possible that oral neoplasm with p16 positive is a separate subtype.Both HPV infection and p16 expression are no correlation with age and gender of the patient,smoking and drinking.
