Objective:To investigate joint procalcitonin with serum endotoxin detection in patients early diagnosis of bacterial infectious diseases.Method:According to the American College of Chest Physicians and Critical Care Medicine Consensus Conference(ACCP/SCCM)diagnostic criteria for screening patients 32 cases,Different levels of infection sepsis classification reference guide is divided into local infection,sepsis,severe sepsis and septic shock group group,And the establishment of non-bacterial infection in the control group 30 cases.ELISA measurement of serum calcitonin in the original inductees PCT,the endotoxin assay application within ELx808 type enzyme linked immunosorbent assay toxin detector,using kinetic turbidimetric Limulus test for quantitative detection.Results criteria:PCT test results:<0.5 ng/ml were negative;?0.5ng/ml as positive.Endotoxin test results:<0.035EU/ml were negative;?0.035EU/ml as positive.Data processing using SPSS 16.0 statistics.Measurement data were normally distributed with±s,skewed distribution with median[M(P25,P75)]FIG.Compare PCT,endotoxin relations with APACHE(acute physiology and chronic health evaluation)? score using Spearman correlation analysis.PCT between different levels of infection were compared using Kruskal Wallis test;endotoxin,APACHE ? scores were compared using ANOVA test.P<0.05 indicates significant difference.Fourfold table ?2 test was used to compare data,diagnostic performance reviews to express the sensitivity and specificity;sensitivity = TP/(TP + FN)×100%,specificity = TN/(TN + FP)×100%.Results:1,3,7 days after admission infection PCT,endotoxin levels and APACHE ? score was significantly higher than that of non-infected group,the difference was statistically significant(P<0.05).2,infection PCT positive results for 29 cases(29/32),the non-infected group PCT positive results for four cases(4/30),the difference was statistically significant(?2 = 37.15,P<0.05).3,in the infected group endotoxin positive results for 28 cases(28/32),the non-infected group endotoxin positive results for 0 cases(0/30),the difference was statistically significant(?2 = 4.07,P<0.05).4,G-bacillus bacteria-infected patients was significantly higher than non-G-bacilli ET-infected patients,Examines the result compared with two groups the difference,the existence statistics significance(P<0.05).5,infection group PCT,endotoxin examination result for masculine 20 example(20/32),PCT,endotoxin examination result for negative 2 example(2/32),PCT examination result result masculine gender also endotoxin for negative 9 example(9/32),PCT examination result negative also endotoxin for masculine 1 example(1/32);PCT in infects in patient's diagnosis sensitivity is 90.6%,the peculiarity is 86.7%;The endotoxin in infects in patient's diagnosis sensitivity is 65.6%,the peculiarity is 60.0%PCT and endotoxin examination two result difference,existence statistics significance(?2 = 6.40,P<0.05).6,PCT levels of patients with different degrees of infection,APACHE II score differences were statistically significant(P<0.05),endotoxin levels had no significant difference(P>0.05),septic shock patients PCT,APACHE II scores were significantly higher than local infection,sepsis and severe sepsis patients.Conclusions:Serum procalcitonin endotoxin increased significantly associated with infection,the detection method of the province,simple,sensitive,can be used as a predictor of early infection,and PCT level change can be identified in critically ill patients in the ICU ward at high risk of having the clinical usefulness of the early diagnosis of infection with high accuracy,specificity,able to effectively distinguish bacterial and non-bacterial infections;measuring endotoxin in the blood can quickly diagnosis of Gram-negative bacterial infections,both bacterial infections can work together to monitor high-risk patients changes in condition,guide rational early empirical anti-infective therapy,timely adjustment of treatment to effectively control and reduce the incidence of nosocomial infections,reduce patient complications and reduce mortality,on the patient's condition,determine the efficacy,prognosis,etc.are significant clinical significance and clinical value.
