Objective:By the application of ECG-Gated 4D-CTA examination on intracranial aneurysm,to discuss the factors associated with intracranial aneurysm rupture.Methods:Patients with intracranial aneurysms who had taken the ECG-Gated 4D-CTA examination in the medical imaging department of Yanbian University Hospital from August 2015 to December 2016 had been collected.Record the patient's age,gender,with or without hypertension history,with or without smoking history.According to subarachnoid hemorrhage or not,the intracranial aneurysms were divided into ruptured intracranial aneurysm(RIA)group and unruptured intracranial aneurysm(UIA)group.Record the location,shape(saccular aneurysm/without saccular aneurysm),the presence or absence of pulsation on the intracranial aneurysm wall and the location and time period.The imaging data that need to be measured include the width of the aneurysm neck,aneurysm height,aneurysm depth,parent-artery diameter of the aneurysm,height and neck ratio(aspect ratio,AR),depth and parent-artery diameter(size ratio,SR),volume of the aneurysm.SPSS 18.0 statistical software was used to analyze the clinical data and imaging data of the two groups,P<0.05 was statistically significant.Results:1.There was no statistically significant difference between the RIA group and the UIA group for the patient's gender,age,smoking history(P>0.05),while the hypertension history is statistically significant between the two groups.Hypertensive patients in the rupture group accounted for 70.8%,while in unruptured group was 33.3%.2.There was no significant difference between the RIA group and the UIA group for the width of the aneurysm neck,aneurysm height,aneurysm depth,parent-artery diameter of the aneurysm,volume of the aneurysm the two groups,while height and neck ratio(aspect ratio,AR),depth and parent-artery diameter(size ratio,SR),shape?pulsation?location of the aneurysms were statistically significant(P<0.05).The mean AR in RIA group and UIA group was 1.41 ± 0.51?0.89 ± 0.40,respectively.The mean SR in RIA group and UIA group was 2.01 ± 0.93?1.28 ± 0.45,respectively.The saccular aneurysms in RIA group and UIA group was 45.8%and 0%respectively.The pulsation of aneurysms in RIA group and UIA group was 83.3%and 13.3%3.respectively.Aneurysms in RIA group were mostly located in the middle cerebral artery,accounting for 28%,and in UIA group were mostly located in the internal carotid artery,accounting for 41.7%.3.By logistic multivariate analysis,AR and pulsation point were associated with ruptured intracranial aneurysms.Conclusion:ECG-Gated 4D-CTA can not only observe the morphological characteristics of intracranial aneurysms,but also to observe the dynamic changes in the aneurysm wall,and height and neck ratio(AR)is higher,the appearance of pulsating points is a high risk factor for predicting aneurysm rupture.
