ObjectiveStudy on dynamic electrocardiogram(DCG)in diagnosis of rapid ventricular arrhythmia patients induced by Premature Ventricular Contraction(PVC),analysed the characteristics of PVC and measured its pre RR interval,coupling interval and their ratio.Explored the relationship between the long short period phenomenon and triggering and maintaining the beget phenomenon of rapid ventricular arrhythmia.Meanwhile,atrial fibrillation has the characteristic of absolute arrhythmia,which can form the long short period phenomenon,analysed whether atrial fibrillation(Af)is closely related to the rapid ventricular arrhythmia.Through getting a deep understanding of the trigger and maintenance mechanism of rapid ventricular arrhythmia,provides more objective reference index for clinical evaluation of ventricular arrhythmia risk classification,and contributes to prevent and control the occurrence of rapid ventricular arrhythmia in clinic.MethodsCollected patients(50 case)who were diagnosed as rapid ventricular arrhythmia by DCG as the observation group,and patients(49 case)who were diagnosed as PVC without rapid ventricular arrhythmia as the control group.Selected the datas of DCG in two group to analyse,and explored the common trigger factors of rapid ventricular arrhythmia.Then measured the pre RR interval,the coupling interval and their ratio of PVC in the two group,then compared their differences.Further more,selected the cases with long short period phenomenon between the two groups and took thestatistical analysis between them to evaluate their correlation.According to the existence of beget phenomenon in observation group was subdivided into two groups,took the statistical analysis between two groups to explore whether rapid ventricular arrhythmia triggerd by long short period phenomenon are more prone to induce the beget phenomenon.Based on the analysis of the characteristics of ECG before and after the occurrence of rapid ventricular arrhythmia,to discuss the mechanism of triggering and maintaining the rapid ventricular arrhythmia.Results(1)There were 531 episodes of rapid ventricular arrhythmia happened in the observation group,490 episode(92.28%)were cansed by PVC.(2)In the observation group,the pre RR interval of PVC was significantly longer than that of the control(909.73+283.22ms?786.63±151.23,P<0.05),and the coupling interval was also longer(541.37+111.49ms,497.01+78.75,P<0.05),but theirratio was significantly smaller than that in the control group(0.63±0.14?0.65±0.11,P<0.05);(3)The incidence rate of short coupling interval(?430ms)in the observation group was slightly lower than that of the control group(13.06%?18.68%,P<0.05);(4)The incidence rate of long short period phenomenon(?735ms)in the observation group was significantly higher than that in control group(70.00%?60.68%,P<0.05);(5)In the observation group,the incidence rate of beget phenomenon in rapid ventricular arrhythmia triggered by long short period phenomenon was higher then that in which without long short period phenomenon(38.00%?21.43%,P<0.05);(6)The number of cases of combined AF in two groups was statistically different and the observation group was more(20.00%?4.08%,P<0.05),including 9 cases of patients with Af formed long short period phenomenon and induced rapid ventricular arrhythmia 52 episodes,the other one occurred ventricular fibrillation repeatedly and form the ventricular electrical storm at 2h after the Af termination.ConclusionPVC is rapid ventricular arrhythmia(ventricular tachycardia,ventricular flutter and ventricular fibrillation)clinically common trigger factors,especially the one with long short period phenomenon has higher rish of triggering rapid ventricular arrhythmia,and the long heart cycle is more obvious,the higher the trigger probability.Meanwhile,the long short period phenomenon also can trigger rapid ventricular arrhythmia to became continuous seizures and form beget phenomenon,thus the risk of malignant ventricular arrhythmia increased.Af due to its absolute arrhythmia,can naturally form a long short period phenomenon to induce rapid ventricular arrhythmia,suggesting that Af may also have the risk of triggering a malignant ventricular arrhythmia.
