| ObjectivePost-Stroke Spastic Paralysis(PSSP)is the focus and difficulty in clinical treatment of stroke.In this study,acupuncture and modern rehabilitation techniques were specifically prescribed to the Fire Filiform Needle Therapy(FFN Therapy)and Bobath Therapy.A prospective randomized controlled clinical study was conducted to observe the multi-level effects of FFN combined with Bobath therapy on patients with PSSP in terms of dystonia,limbs’ motor function and the ability of activities of daily living(ADL).Trying to explore a better-efficacy,more operable,more affordable,optimal rehabilitation program,and to provide evidence-based medicine(EBM)basis for PSSP.To further improve the overall treatment effects,reduce the patient’s pain and dysfunction,improve the quality of life of patients.MethodsAccording to the diagnostic criteria of stroke stipulated by CHINESE SOCIETY OF NEUROLOGY in 2007,80 stroke patients who met the inclusion criteria were selected,and divided randomly into Intervention group(FFN combined with Bobath Therapy group)and Controlled group(simple Bobath Therapy group)by random number table.Each group included 40 patients.The treatment of two groups were divided into two courses,each of which lasted for 2 weeks.The treatment was manipulated once a day(except Sunday).Before and after the whole treatment,① the modified Ashworth Scale(MAS)was used to assess the dystonia of patients;② the simplified Fugl-Meyer Assessment(FMA)was used to evaluate limbs’ motor function;③ the modified Barthel Index(MBI)was used to assess the ability of ADL.And it is required to observe the degree of improvement of dystonia,motor dysfunction and daily-life ability of patients,and finally to evaluate the therapeutic effect and safety of two different teratments.ResultsA total of 80 cases were collected in the clinical study.In the whole trial and treatment process,a total of 3 people shed doff.Intervention group off 2 people,the control group off 1 person.(1)Comparison of baseline:① The difference in gender of patients in the two groups was measured by chi-square test,χ2=0.1225,P=0.7263>0.05.②The difference in age of patients in the two groups was measured by t test,t=0.2288,P=0.8196>0.05.Therefore,there is no statistically significant difference of gender and age between the two groups.③ The difference in course of disease and the MAS rating,FMA score,MBI score of patients before the treatment in the two groups was measured by statistical tests,P>0.05,Therefore,there is no statistically significant difference of patients’condition between the two groups.In short,the difference of gender,age,patient’s condition between the two groups is not obvious and statistically significant.The two groups are comparable.(2)Comparison of efficacy:① the total effective rate:The patients’FMA score was used as the standard.In the Intervention group:Control led group,8:5 cases were cured,17:9 cases were effective,8:18 cases were progressive,5:7 cases were ineffective and the total effective rate was 86.84%:82.05%.The rank sum test show:u=-2.1095,P=0.0349<0.05.the difference he two groups was statistically significant.② Before and after the treatment,the difference of MAS rating,FMA score and MBI score were statistically significant(P<0.05)in each group.③ After the treatment,the difference of MAS rating,FMA score,MBI score and increased scores were significantly between the two groups(P<0.05).Conclusion(1)FFN combined with Bobath therapy and simple Bobath therapy all can effectively improve the patient’s paralyzed limb spasm,improve limb motor function and daily living ability.(2)The total effective rate of FFN combined with Bobath therapy is higher than simple Bobath therapy,which can better relieve muscular spasm and improve the function of the limbs and the activities of daily living.(3)FFN combined with Bobath therapy is a safe and effective therapy that can optimize the rehabilitation for PSSP. |
