Objective: retrospective analyse application of 3D printing technology in the diagnose and treatment of Pilon fractures,to explore the clinical value of 3D printing technology in Pilon fractures.Methods: 26 cases of Pilon fractures were collected from June 2014 to June 2016 in the affiliate Rui Kang hospital of TCM university of Guangxi.12 patients were applied 3D printing technology to print a solid model of Pilon fracture,according to the solid model to be familiarized with the type of fracture,imitated surgery,designded surgical planing,then operated with routine surgery incision reduction and internal fixation,they were setted into 3D group.Another 14 cases of Pilon fractures were designded surgical planin accordeding to X-ray and CT only,then also operated with routine surgery incision reduction and internal fixation,they were setted into control group.After surgery,according to the observation and comparision of the time of surgery,surgical bleeding volume,surgery fracture reduce degree,ankle function of the two groups and early stage complication,to evaluate the clinical value of 3D printing technology in Pilon fractures.Results: under the equal conditions,the same surgery group operated all the patiens.The 3D group: the average time of surgery was165.65±24.58 min,The control group: the average time of surgery was216.38±32.77 min,;The 3D group: the average surgical bleeding volume were250.83±42.22 ml.The control group: the average surgical bleeding volume were 256.57±42.30 ml.The surgery fracture reduce degree: The 3D group:anatomical reduction 9 cases,kind reduction 3 cases,bad reduction 0 cases,anatomical reduction rate is 75%;The control group: anatomical reduction 5cases,kind reduction 6 cases,bad reduction 3 cases,anatomical reduction rate is 35.72%.The surgery ankle function(after 1 month): the 3D group: excellent9 cases,good 2 cases,fair 1 case,bad 0 case,the excellent and good rat is91.67%;the control group: excellent 5 cases,good 3 cases,fair 3 case,bad 3case,the excellent and good rat is 57.14%;The surgery ankle function(after 3month): the 3D group: excellent 10 cases,good 2 cases,fair 0 case,bad 0 case,the excellent and good rat is 100%;the control group: excellent 5 cases,good4 cases,fair 2 case,bad 3 case,the excellent and good rat is 64.29%;The surgery ankle function(after 6 month): the 3D group: excellent 10 cases,good2 cases,fair 0 case,bad 0 case,the excellent and good rat is 100%;the control group: excellent 5 cases,good 5 cases,fair 2 case,bad 2 case,the excellent and good rat is 71.43%;The early stage complication(after surgery): the 3D group: skin necrotize 1 cases,wound infection 1 cases,fracture second displacement 0 case,complication rat is 16.67%;the control group: skinnecrotize 2 cases,wound infection 2 cases,fracture second displacement 1case,complication rat is 35.72%;The surgery time and early stage complication(after surgery)of the 3D group are more less than that of the control group,the surgery fracture reduce degree and ankle function of 3D group are better than the control group,two groups of significant differences(P < 0.05),with statistical significance;the surgical bleeding volume of 3D group is close to the group of the control,significant differences(P >0.05),no statistical significance.Conclusion: In the Pilon fracture diagnose and treatment using 3D printing technology with the clinical value significantly,surgeon can imitate operation and familiarize surgical process,not only can effectively reduce operation time and early stage complication(after surgery),but also can improve the surgery fracture reduce degree and ankle function,it is worth applying in clinical.
