Objective:To analyze the clinical application and complications of single port laparoscopic surgery in Department of Urology,and to provide a reference basis for the selection of safe and effective treatment techniques for patients in Department of Urology.Methods:Ninety-eight cases of Department of Urology in The People's Hospital of Langfang City from September 2014 to September 2016 were selected.Among them,26 cases were treated with radical nephrectomy,and the other was treated with ureteroscopic lithotripsy in 24 cases,and removal of renal cysts in 48 cases.All patients were divided into observation group(single hole laparoscopic)in 49 cases,control group(traditional laparoscopic)in 49 cases.Observation the patients in each group:operation time,incision size,estimated blood loss,the mean value of airway pressure,the time of removal of endotracheal tube after operation and the decrease of serum hemoglobin after operation,the number of days after hospitalization,visual analogue scale(VAS)score,classification of postoperative complications,incidence of subcutaneous emphysema.Results:1)Two groups of patients with radical resection of renal tumor with operation time,incision size,estimated blood loss,the decrease of serum hemoglobin after operation,the number of days after hospitalization,visual analogue scale(VAS)score,classification of postoperative complications,there were no statistically significant differences(P>0.05).The mean value of airway pressure(21.32±3.87)cmH2O was significantly lower than that in the control group(27.98±4.67)cmH2O;the time of removal of endotracheal tube(19.79±2.36)min after operation was significantly lower than that of control group(34.23±3.11)min.The incidence of subcutaneous emphysema was 15.38%lower than that of the control group30.77%,and the difference was statistically significant(P<0.05).2)Two groups of patients with ureterolithotomy with operation time,estimated blood loss,the decrease of serum hemoglobin after operation,the number of days after hospitalization,visual analogue scale(VAS)score,classification of postoperative complications,there were no statistically significant differences(P>0.05).Single incision laparoscopic observation group incision(1.83±0.31)cm,which was significantly shorter than the control group(3.15±0.52)cm;mean airway pressure(18.77±2.25)cmH2O was significantly lower than that of the control group(23.92±4.13)cmH2O;the time of removal of endotracheal tube(15.36±3.78)min after operation was significantly lower than that of control group(34.23±4.67)min;the incidence of subcutaneous emphysema was 8.33%lower than that of the control group,and the difference was statistically significant 33.33%,and the difference was statistically significant(P<0.05).3)Two groups of patients with renal cyst removal with operation time,estimated blood loss,the decrease of serum hemoglobin after operation,the number of days after hospitalization,visual analogue scale(VAS)score,classification of postoperative complications,there were no statistically significant differences(P>0.05).Single incision laparoscopic observation group incision(1.46±0.54)cm,which was significantly shorter than the control group(3.02±0.67).cm;mean airway pressure(17.899±2.47)cmH2O was significantly lower than that of the control group(21.95±3.35)cmH2O;the time of removal of endotracheal tube(24.68±7.99)min after operation was significantly lower than that of control group(44.23±4.82)min;the incidence of subcutaneous emphysema was 0 lower than that of the control group(16.67%),and the difference was statistically significant(P<0.05).Conclusions:The single port laparoscopic Department of urology surgery is safe and reliable,and can reduce the incidence of subcutaneous emphysema,reduce the possibility of patient's airway damage,shorten the anesthesia recovery time,and improve the appearance but also has a better therapeutic effect,has certain clinical application value,and is worth promoting.
