The Analysis Of PSA And DRE Combined With MRI In Prostate Biopsy

ObjectiveBy analyzing the correlation between PSA?MRI?DRE and the positive rate of prostate biopsy,to explore the guiding significance of PSA?MRI?DRE to prostate biopsy,provides a reference for clinical.MethodsRetrospectively analyzed the clinical data of 770 patients undergoing transrectal ultrasound-guided prostate biopsy from January 2012 to December 2015.The Patients was distinguished by PSA,named<4ng/ml,4-10ng/ml,10-20ng/ml,>20ng/ml,analyzing the relationship between the MRI?DRE and the positive rate of prostate biopsy in each PSA group.SPSS statistics 20 software were used to analyze the data.ResultsPositive rate of prostate biopsy was increased with the PSA level increase,the positive MRI or DRE examination has a higher positive rate of prostate biopsy than the negative(p<0.01).when PSA<4ng/ml,the positive puncture rate in positive MRI or DRE was 19.3%and 28.6%respectively,has no significant difference with the overall positive puncture rate(p=0.79,p=0.15).The positive puncture rate was 50.0%when MRI and DRE are all positive,higher than MRI positive(p=0.03).When PSA was 4-10ng/ml,the positive puncture rate in positive MRI or DRE was 38.1%and 34.7%respectively,higher than the overall positive puncture rate(p<0.01,p=0.02),the positive puncture rate was 56.8%when MRI and DRE are all positive,higher than MRI or DRE positive(p<0.01,p=0.03).When PSA was 10-20ng/ml,the positive puncture rate in positive MRI or DRE was 37.1%and 43.5%respectively,higher than the overall positive puncture rate(p<0.01,p=0.01),the positive puncture rate was 48.7%when MRI and DRE are all positive,has no significant difference with MRI or DRE positive(p=0.14,p=0.62).When PSA>20ng/ml,the positive puncture rate in positive MRI or DRE was 80.4%and 82.9%respectively,higher than the overall positive puncture rate(p<0.01,p<0.01),the positive puncture rate was 90.3%when MRI and DRE are all positive,higher than MRI positive(p=0.01),has no significant difference with DRE positive(p=0.06).ConclusionWhen PSA<4ng/ml,MRI or DRE examination can't effectively increase the puncture positive rate.When PSA was 4-10ng/ml,MRI or DRE examination can effectively increase the puncture positive rate,MRI/DRE positive patients should do further DRE/MRI examination.When PSA was 10-20ng/ml,MRI or DRE examination can effectively increase the puncture positive rate,further DRE examination should be done in MRI negative patients.When PSA>20ng/ml,the puncture positive rate was very high and MRI or DRE examination can effectively increase the puncture positive rate,further DRE examination should be done in MRI positive patients,further MRI examination should be done in DRE negative patients.
