The Clinical Research About Treating The Multiple Drug Resistance Of Bacterial Pneumonia By The Reinforce Healthy Qi And Purge Toxins

ObjectiveSupporting by the method of randomized controlled trial to observing the efficacy and safety of the treatment of multiple drug-resistant bacterial pneumonia by reinforce healthy qi and purge toxins, to exploring the theoretical basis about the treatment of multiple drug-resistant bacterial pneumonia by reinforce healthy qi and purge toxins and to providing fundamental basis for treatment this disease by reinforce healthy qi and purge toxins.Methods1.Theoretical research:Review the similar research and understanding of the traditional Chinese medicine on "dyspnea", "cough" and "lung fever" and so on, summarizing the multiple drug-resistant bacterial pneumonia with the modern medicine. To exploring the theory of the traditional Chinese medicine about the multiple drug-resistant bacterial pneumonia, to exploring the theoretical and practical basis about treating the multiple drug resistance of bacterial pneumonia by reinforce healthy qi and purge toxins.2. Clinical research:Choose the patients in Chengdu University of traditional Chinese medicine hospital EICU and emergency between January 2014 and February 2016, they were randomly divided into experimental group and control group; the control group treats with the western medicine by the pneumonia and multiple drug resistance to prevent and control related guide, the experimental group treats with the methods of Traditional Chinese Medicinal broth on the basis of control group. As the evaluation time point on the treatment of before and3,7,14 days, observing the inflammation index:Routine blood, C-reactive protein (CRP), Procalcitonin(PCT); lung injury parameters:the clinical pulmonary infection score (CPIS); organ dysfunction index:acute physiology and chronic health evaluation (APACHE ?) and systemic infection associated organ failure score (SOFA score); case fatality rate of 14 days;the incidence of invasive mechanical ventilation; the clearance of the drug-resistant pathogens; the curative effect of traditional Chinese medicine and Chinese medicine symptom integral.etc.Results:1. The baseline survey:Compare the two groups of the baseline information, differences had no statistical significance (P>0.05).2. The security Index:The adverse reaction rates of the experimental group and the control group were 12.8% and 7.7%. Compared with the adverse reaction rates in the two groups, there was no statistically significant difference (P> 0.05).3. Outcome indicators:The experimental group and the control group's total effective rate are 87.2% and 66.7%, the clearance rate of the Drug-resistant bacteria are 84.6% and 64.1%, the incidence of invasive mechanical ventilation are 10.3% and 10.3%, there was statistically significant difference (P<0.05), the mortality of the two groups'of patients are 7.7% and 15.4%, there was statistically significant difference (P>0.05).Comparing the two groups of patients with Chinese medicine symptom integral, inflammation (WBC, NEUT%, CRP and PCT), lung injury and organ function index score (CPIS) score (APACHE II grade, SOFA), before and 3 days treatment, there was no statistically significant difference (P>0.05), there are statistically significant difference in treatment for 7 days and 14 days (P<0.05).Conclusions:1.The theory of the reinforce healthy qi and purge toxins can be suitable for clinical treatment of multiple drug-resistant bacterial pneumonia, the principles of treatment according to reinforce healthy qi and purge toxins conform to the basic theory of TCM; based on the supporting method of traditional Chinese medicine prescription is stand on the safe side, less adverse reaction.2.Compared with control group, the method of combined Chinese and western medicine treatment which is supporting by the reinforce healthy qi and purge toxins can effectively improve the therapeutic effect of TCM and the clearance rate of Multiple-resistant strain about the patients with multiple drug resistance bacterial pneumonia; can effectively reduce the patient's level of inflammation, lung injury score, organ failure score and the incidence of invasive mechanical ventilation index, witch have curative effect advantage.
