ObjectiveIn order to explore the factors influencing the severity and prognosis of ACI, we gathered the clinical parameters of ACI patients from a 3A degree hospital and then performed an analysis. The outcome of this study can provide a scientific basis for enhancing the accuracy of diagnosis and treatment, and furtherly providethe inventions for high-risk groups. Therefore we can control the occurrence and development of ACI effectively and improve the prognosis of patients with ACI.MethodsACI patients were selected from January 2011 to Jun 2016 in the Department of Neurology of a 3A hospital in Qiqihar City. The basic demographic characteristics, past history, living habits, other clinical characteristics and important laboratory examination outcomes were all collected and then analyzed by SPSS 21.0. The data were analyzed by statistical methods including t-test,Chi-square test, two sample rank test and logistic analysis of regression.Result1. A total of 814 ACI patients were studied, 626 patientswere first cases(76.90%) and 188 were recurrent patients(23.10%). The number of mild patients was 204 cases(25.06%), while moderate patients and serious patients were 610 cases(74.94%). In addition, 55 patients died and 759 patientsleft hospital.2. Of all the patients,429(52.7%) cases were male and 385(47.30%) cases were female.The difference between the severity and prognosis of patients were not significant with different gender(P> 0.05). The age of the patients ranged from 31 to 92, and the average age was 63.18 ± 12.02 years, and the most patients were above 60 years old, accounting for 57.49%. Of all the patients, 482 patients had a normal weight, accounting for 59.21%. There was no significant difference in the severity and prognosis of the patients of different ages(P> 0.05). There was significant difference in patients with different BMI groups and different occupational groups(P<0.05).3.269 patients(33.05%) had a history of hypertension. The patients with hypertension had worse prognosis than those without hypertension(P <0.05), but hypertension had no effect on the severity of ACI(P>0.05). 3 patients(0.37%) had a family genetic history, 89 patients(10.93%) had a diabetes history, 272 patients(33.42%) had a cardiovascular history or cerebrovascular history. Whether or not have a family history, diabetes mellitus or cardiovascular or cerebrovascular history, there was indifference in the seriousness and prognosis of the disease.(P> 0.05).4.Of all the patients, 181 cases(22.23%) had a history of smoking and 142 cases(17.44%) had a history of alcoholintake. Whether smoking or drinking had no effect on the disease severity and prognosis of ACI patients(P> 0.05).5.The levels of occupation, BMI, years of schooling, K+, FIB, BUN, AST, LDH, PLT and GGT were correlated with the severity of ACI(P<0.05).6.336 patients(41.28%) had carotid stenosis. The number of lesions was mainly multi-focus. There were 225(27.64%) patients with single lesion and 589(72.36%) patients with multiple lesions. Of all the clinical types, patients with lacunar infarction were the most, 353 cases, accounting for 43.37%. Followed by 246 cases(30.22%) with cerebral thrombosis, 215 cases(26.41%) with cerebral infarction. Carotid stenosis or not, the number of lesions and different clinical types had no effect on disease severity and prognosis of ACI patients(P> 0.05).7.766 patients(94.10%) had paid hospital expenses through social insurance.The annual income, years of schooling, time of onset to admission, hospital day and way of payment had no effect on the disease severity and prognosis of patients(P> 0.05).Conclusion1. The main patients ofthe 814 are over 60 yearsold. The number of men are slightly greater than women. Most patients have no permanent occupation and the majority of the patients are married. The BMI value of the most patients are normal. The education years mainly concentrated in 6 to 8 years or less than 6 years and the income are mainly focus on 10000 to 50000. Sex, age, annual income and other demographic characteristics do not affect the patient's diseases condition and prognosis.2.Patients who developed acute cerebral infarction often supervened with a history of hypertension, cardiovascular history, but patients with a history of glycuresis or a family history are less. Few patients have a history of unhealthy habits including smoking and drinking.3. The majority of the 814 ACI cases are patients with first onset, and the ratio of carotid artery stenosis are slightly greater than normal people. The time from the onset to hospital are all concentrate on 24 hours. Of all the ACI, patients with lacunar infarction are the most, and the majority of patients are multiple lesions. What's more, most of the patients paid their hospitalization expenses through medical insurance for urban workers.4. The changes of occupation,BMI,years of schooling,FIB, BUN, AST, PLT, GGT,LDH and K+ levels are related to the severity of illness of ACI.5. Hypertension history asa important factor is impacting the prognosis. The demographic characteristic, living habit, vital signs on admission, index of laboratory examination, clinical features and payment manner of medical expenses all have no significant effect on the prognosis.
