ObjectiveTo compare the recent clinical efficacy of circular suture and modified sturmdorf suture in the cervical cold knife conization. Methods420 cases of patients suffering from high-grade squamous intraepithelial lesion(CIN2-3) who underwent cold knife conization in our hospital from June 2014 to June 2015 were randomly selected, including 200 cases as the study group with circular suture on the cervical wound and 220 cases as the control group with improved sturmdorf suture. The operative time, intraoperative blood loss, the height and diameter in the cervical conization, postoperative hospital staying, the rate of postoperative infection in the hospital, postoperative blood loss more than 2 times menstrual quantity needing medical care intervention and cervical stenosis; Outpatient care of 6 months from postoperative(follow-up rate 100%): pelvic infetion rate, wound healing, degree of satisfaction of the cervical restore, the rate of cervical mucous membrane hyperplasia, recurrence and residual lesion. ResultThe operative time( 26.9±2.9min) and the intraoperative blood loss(17.1±3.4ml)in the study group were less than the operative time(38.1±2.8min)and the intraoperative blood loss(24.0±3.2ml)in the control group, which exhibited significant differences(P<0.05).3 case of postoperative blood loss more than 2 times menstrual quantity appeared in the study group(2 cases –operative hemostasis,again;1 case-hemostasis by medicine and compression, return in hospital). 11 cases appeared in the control group( 4 cases –operative hemostasis, again; 2 cases- hemostasis by interventional operation;5case- hemostasis by medicine and compression, return in hospital),which also exhibited significant differences(P<0.05). The height and diameter in the cervical conization and postoperative hospital staying showed no significant statistical differences(P>0.05).Each of the two groups appeared 1 case of cervical stenosis. The control group appeared 1 case of cervical mucous membrane hyperplasia, while the study group not appeared, which showed no significant statistical differences(P>0.05). There were 3 cases pelvic infection in the study group, while 4 cases in the control group, which showed no significant statistical differences(P > 0.05). For all patients,no postoperative infection, recurrence, residual lesion were observed. ConclusionConization of cervical wounds are treated with circular suture to shorten the operative time obviously, reduce the intraoperative and postoperative blood loss effectively, not prolong postoperative hospital staying and not increase the rate of postoperative infection, recurrence and residual lesion. It is well worthy of clinical promotion.
