The Anorectal Dynamics Influence And Clinical Study Of Regulating-qi And Clearing-heat Decoction With Biofeedback In Qi Stagnation And Intestinal Dryness Pattern Of Irritable Bowel Syndrome With Constipation

Objective: To observe the anorectal dynamics influence and clinical obs ervation of regulating-qi and clearing-heat decoction and their joint biofeedback in treating qi stagnation and intestinal dryness pattern of irritable bowel syndrome with constipation(IBS-C).Methods: Totally 120 selected patients with qi stagnation and intestinal dryness pattern of irritable bowel syndrome with constipation were rand omized into Chinese medicine group,combination-therapy group,and West ern medicine group.The Chinese medicine group was given with regulating-qi and clearing-heat decoction,and the combination-therapy medicatio n group was given with regulating-qi and clearing-heat decoction and biofeedback treatment,while the Western medicine group was prescribed with permeable laxative of lactulose oral solution.The course of treatment for all groups was 4 weeks.All groups were underwent anorectal mano metry before and after the treatment,while the changes of anorectal dynamics index and main symptoms were observed.Results:(1)After treatment,the total effective rate of the combination-the rapy group,the Chinese medicine group and the Western medicine group were 92.3%,87.5% and 82.5% respectively.The curative effect of the co mbination-therapy group was better than another two groups(P<0.05);A nd the Chinese medicine group was better than the Western medicine group(P<0.05).(2)After treatment,the clinical symptom scores of the three groups were better than before treatment(P<0.01);And the combination-t herapy group was better than another two groups(P<0.05);The Chinese medicine group was better than the Western medicine group(P<0.05).(3)The defecation frequency,fecal character and abdominal distension of the combination-therapy group were better the Chinese medicine group(P<0.05).All the clinical symptom scores of the combination-therapy group were better than the Western medicine group(P<0.05).The fecal character,a bdominal pain,belching hiccup,dry throat,bitter taste of the Chinese medi cine group were better than the Western medicine group(P<0.05).All clinical symptom scores of the combination-therapy group and Chinese medicine group were better than before treatment(P<0.01 or P<0.05).The defecation frequency,fecal character,abdominal pain,abdominal distension of the Western medicine group were better than before treatment(P<0.01 or P<0.05).(4)Comparing the anorectal manometry sign,after treatment,the three groups had no significant difference in anal sphincter resting pressure(P>0.05).The maximal squeeze pressure,rectal defecation pressure,anal sphincter relaxation rate,sensory threshold,initial sensation volume,rectal maximal capacity of the combination-therapy group were better than another two groups(P<0.05).The maximal squeeze pressure,anal sphincter relaxation rate,sensory threshold,initial sensation volume,rectal maximal capacity of the Chinese medicine group were better than the Western medicine group(P<0.05).The maximal squeeze pressure,rectal defecation pressure,anal sphincter relaxation rate,sensory threshold,initial s ensation volume,rectal maximal capacity of the three groups were better than before treatment(P<0.01 or P<0.05).Conclusions:(1)Regulating-qi and clearing-heat decoction is to soothing liver,regulatingqi,relieving qi-depression,and clear away gallbladder,dredging stasis fu-organs,relaxing bowels.Single the parties and their joint biof eedback could improve the clinical symptoms in qi stagnation and intes tinal dryness pattern of irritable bowel syndrome with constipation,and their clinical efficacies are significant.Regulating-qi and clearing-heat dec oction and their joint biofeedback are effective Chinese medicine prescription andapplication,and they are worthy of being extended to clinical.(2)Regulating-qi and clearing-heat decoction and their joint biofeedback could improve the anorectal dynamics index in qi stagnation and intestinal dryness pattern of irritable bowel syndrome with constipation.
