Background of researchAlong with the development of the socialist market economic system,medical care is opening the door to all the people.Medical services will meet taxpayer's requests by its multi operation form.However,incomplete medical system brings illegal fortunes to minority groups and persons by unlicensed doctors and medical staff,out of clinic scope,ineffective disinfection instrument and improper medical care waste.The influence of illegal activities in medical care market is challenging the benefits of taxpayers.The main medical care that most of the publicans are currently choosing is the primary medical institutions.These institutions are the essential parts for the all different kinds of medical services.They take the load of most of the basic medical service and basic public service,which makes it directly related to most publicans' medical status.It is highly related to taxpayers' benefits that how many unlicensed doctors are currently working for the primary medical institutions,and whether the primary medical institutions are able to operate lawfully.Therefore,it has significant meaning for both of the primary medical care market and the publicans' safety that the governments take actions to strengthen the inspection of the primary medical institutions,and to standardize the primary medical professions.One of the most efficient means to provide expected inspection to the primary medical institutions is for the medical service department of the city governments to visit the medical service anonymously.Throughout the data that is collected by the under covered agents,it becomes easier to collect the unlawful activities that the primary medical institutions are currently having.When these activities have been collected,it is possible to bring up certain policies for the change accordingly.Meanwhile it can be used as sample cases for the medical service department and inspection department to strengthen and improve the inspection for the institutions.Purpose of researchThis study is to analyses Illegality by visiting primary medical institutions anonymously and involved medical institutions during the year of 2013 to 2015,and put forward some countermeasures according to existing problems.Materials and MethodsMaterials come from the archived data of under-cover investigations in the jurisdictions during the year 2013-2015.There are 303 medical registered institutes in three years,including 72 institutes in 2013,84 institutes in 2014,and 147 institutes in 2015.Sort the materials,input the data to form database and use the method of SPSS.Statistical indicators adopt percentage and proportion.Date input twosome to ensure accuracy and double check one by one.Key outcomes1.Based on the under-covered investigation,there are up to 48.8%of the medical staff who currently working for the primary medical institutions without licenses.2.Based on the under-covered investigation,illegitimacy proportions are 25.8%of village clinic,18.1%of western medicine clinic,and 14.2%of clinic.3.Based on the under-covered investigation,there are up to 48.4%of medical personnels without any formal qualifications,which mainly distributes clinics,community health service institution,western medicine clinic,Chinese and western medicine clinic and dental clinic.Meanwhile,the serious issues of western medicine clinic and community health service institution is out of scope,and the illegality rate of both western medicine clinic and community health service institution are 28.6%.4.Based on the under-covered investigation,there are up to 68.40%of unqualified writing prescriptions in the primary medical institutions to the exclusion of township hospitals and medical cosmetic clinics.5.Based on the under-covered investigation,there are up to 82.6%of illegal disinfectant management and 80.6%of illegal medicine waste treatments in primary medical institutions.Village clinics and clinics are the high incidence areas.6.Based on the under-covered investigation,41.2%of dental clinics and 11.2%of massage institutions have the serious issues of medical Practices without licenses.7.Illegal propagandas are widely found in drugstores,life cosmetology institutions,and massage institutions.Inorder,the illegality rate of above institutions is 100%,83.3%and 82.3%.8.Based on the under-covered investigation,70.9%of medical practices without legal permission mainly take place in region of towns and townships.Countermeasure and Suggestions1.Focus on those towns where Illegitimacy dental clinic and medical practice without legal permission exist frequently to play both defense and blocked sparse.2.Straighten up local hygiene supervise organization system,and allocate the scientific and rational hygiene supervisors.3.Strengthen disinfectant management and medical waste treatment management.4.With bad practice behavior scoring management as the starting point,strengthen supervision of the activities and institutions with major violations.5.Strengthen specialty training of primary care staff to improve lawful practice awareness.6.Strictly control permit approval and strengthen construction of standardized clinics and the lawful practice of publicity platform.7.To further establish and improve the medical and health laws and regulations and the basic law of health as soon as possible.8.Strengthen basic medical knowledge and medical and health laws and regulations to spread.
