ObjectiveRetired cadres in Shenyang were chosen as the research objects and prospective cohort study design was used to investigate whether hyperuricemia in this population had an effect on the onset of diabetes.It meant to provide certain clinical value for the etiology of elderly diabetes and give the effectively prevention and control measures for the elderly population.MethodFrom September 2010 to September 2015,retired cadres who had physical examination at the Central Hospital Affiliated to Shenyang Medical College were in the cohort.Dynamic cohort study design was used and 5 years were followed up.The indexes of demographic characteristics were recorded each year.Such as age,gender,height,weight,smoking,drinking,physical exercise and the way of life.Laboratory examination such as SBP,DBP,SUA,FBG,TG,TC,HDL-C,LDL-C were also recorded.It was end with someone who got DM.Epidata3.1 was used for data entry and SPSS23.0 was used to analysis.Result1.Through September 2010 to September 2015,715 subjects enrolled in the cohort first,including 608 males and 107 females and the mean age were 79.40±6.12 years old.In the baseline,the current smokers were accounted for 17.2%,drinkers were 28.4%,usually exercise were 45.9%.By asking if he or she had a DM family history,only 7.7%of the population had.The average value of BMI was(24.08 + 2.80)kg/m2.Finally we divided this people into SUA normal group(536)and HUA group(179).2.The total number of person years of exposure were 3321.5 years in 2010 to 2015.After 5 years' follow-up,a total of 76 people had DM and the incidence density was 22.88/1000 person-years.3.After exploring the influence between smoking,drinking,family history,hyper tension,hyperlipidemia and the incidence of DM,it was found that the risk of DM was significantly higher in patients with family history of DM disease than those without(RR=3.773,95%CI:2.200-6.7472;P<0.001).4.By comparing the incidence of diabetes mellitus among different characteristics.In different sex groups,it found that the number of male DM patients was 64,the incidence density was 22.72/1000 person-years.And for the number of women,the incidence of DM was 12,the incidence density was 23.76/1000 person-years.There was no statistically significant difference between the two groups(P=0.885).To compare the different age groups,the number of the incidence of DM was 35 in 80 years' groups,the incidence density was 26.18/1000 person-years.The incidence of DM in the other group was 20.66/1000 person-years.There were no significant differences between the two groups(P=0.296).At the end of follow-up,cumulative incidence of DM in group HUA was 31,the cumulative observed person were 825 person-years,the incidence density was 37.58/1000 person-years.The number of DM in SUA normal group was 45,the cumulative observed person were 2496.5 person-years and the incidence density was 18.03/1000 person years.DM incidence density of HUA group was higher than that in normal SUA group,the difference was significant(P=0.001).RR between two group was 2.084(95%CI,1.259-3.532).5.GEE model was used to analyze the risk factors for DM.When compared with normal group,the difference in group HUA was statistically significant(P=0.013),the relative risk was 1.949(95%CI,1.152-3.298).DM family his tory compared with no family history,the risk of DM significantly increased(RR=26.895,95%CI:5.301-136.444;P<0.001).The risk to people who had hyper lipidemia was significantly increased(RR=3.346,95%CI:1.856-6.034;P<0.001).Conclusion1.After five years' follow-up,the incidence density of DM was as high as 22.88/1000 person years.The results showed that the incidence of DM among retired cadres was higher in Shenyang.2.DM family history and hyperlipidemia were important risk factors for the onset of DM.The family history of risk to DM incidence was 26.9 times higher than those without.The people who had hyperlipidemia had 3.3 times higher risk than those who hadn't.The elderly population who had family history of diabetes and hyperlipidemia should be pay more attention.lt helped to reduce the occurrence and development of diabetes.3.By multivariate analysis,the risk of DM in HUA group was about 1.949 times higher than that of normal SUA group.HUA was an independent risk factor for DM.Prevention and treatment of hyperuricemia had a positive effect on the prevention of diabetes.
