Benign prostatic hyperplasia(BPH)is the most common disease in the over middle-aged men, which induces dysuria.BPH is known to adversely affect the quality of life of around one-third of men over the age of 50.Almost all men will develop the histological BPH by the age of 80.The prevalence is different due to regionals, ethnics, participants and study methods. Many factors can affect BPH.Age and hormones are currently recognized as risk factors for BPH.The influence of other factors such as smoking, drinking, physical activity on BPH is still controversial. At present, there is no epidemiological information on BPH in Henan Province. We conducted cross sectional study of BPH in men 40 years old or older in Zhengzhou rural area, cooperated with the third People's Hospital of Zhengzhou, with the assistance of Xinmi Chinese Medicine Hospital. We investigated the prevalence rate of BPH,the correlation with epidemiologic factors and the impact of genetic factor on BPH.ObjectiveTo understand the prevalence of BPH in men 40 years old or older in Zhengzhou rural area through a cross-sectional study and analyzed the correlation with epidemiologic factors and the effect of genetic factor on BPH.MethodsA multistage and cluster sampling method was used to randomly select the male respondents in Zhengzhou rural area.Man who was 40 years old or older carried out the questionnaire survey and related anthropometric and laboratory examination.According to clinical diagnosis, the cases and controls were preliminary selected. Then the first-degree relatives of cases and controls conducted the same investigation.Logistic regression was used to analyze the association between life style, disease history, biochemical indicators and BPH.The data of first-degree relatives of cases and controls was used to calculate the heritability. Results 1.Of the 1043 men that were investigated,976 were included in the analysis. The overall response rate was 93.58%.A total of 98 men met BPH diagnostic criteria and the prevalence rate was 10.04%.The prevalence increased with age(?2=64.35,p<0.05).2.The prevalence rate of LUTS was 29.20%.The most common symptom was incomplete emptying(36.99%),followed by nocturia(31.15%),frequency(30.02%)and weak stream(29.20%).The severity of LUTS was positively correlated with age(r=0.386,p<0.05).There was a positive association between I-PSS and Qo L(r=0.759,p<0.05).3.The results of logistic regression after adjusted age showed that prostatitis,PSA were related to BPH,and corresponding ORs with 95%CI s were 2.00(1.06-3.81),6.81(3.68-12.62),respectively.Marital status, smoking, drinking, sexual activity, eating habits, overweight or obesity, waist circumference, physical exercise, hypertension, diabetes were irrelevant to BPH.4.According to the prevalence of first-degree relatives between case group and control group, the heritability of BPH was 40.48%?Conclusions 1.The prevalence rate of BPH in men 40 years old or older in Zhengzhou rural area was lower than that in other rural areas in China.2.The prevalence of LUTS was consistent with that of other rural areas in China. The higher I-PSS score, the greater impact of LUTS on life.3.Age,PSA and prostatitis were related to BPH.Lifestyle, eating habits and disease history had nothing to do with BPH.4.The genetic factor played a certain role in the occurrence of BPH.
