ObjectiveThe purpose of this study is to evaluate the effect of community grading management of dyslipidemia in Hangzhou from 2014 to 2015,analyze influencing factors of dyslipidemia community management,and explore the useful method of dyslipidemia management.Based on that,provide a reference for more comprehensive and effective community comprehensive management methods for dyslipidemia.Methods1.Sample:Using multiple stage sampling method,2498 patients were selected as intervention group from the 25577 patients who were enrolled in the pilot project of ommunity grading management of dyslipidemia in Hangzhou at the proportioning of 10%,and another 1020 patients who were not enrolled in the project were selected as control group.2.This study analyzed the effect of community grading management of dyslipidemia,mainly including the level of blood lipid before and after intervention,the standard rate of dyslipidemia management,and the influencing factors of management effect.3.Statistical analyses were carried out using SPSS 17.0 for Windows.Statistical methods used included general statistical description(such as mean and standard deviation),Pearson chi-square test,independent sample t test,paired sample t test,single factor non conditional logistic regression analysis,and multi factor non conditional logistic regression analysis.Results1.There was no significant difference in the demographic characteristics between the intervention group and the control group,the contract rate with general practitioner of the intervention group was significantly higher than that of the control group.2.The intervention group and the control group had no statistically difference in lipid levels of the base line.After intervention,the intervention groups' TC,TG,LDL-C levels significantly reduced conparing to the base line,and was significantly lower than the control group,but its HDL-C level had no significant differences with the base line.3.The baseline compliance rate of the intervention group and the control group had no statistically difference.After intervention,the intervention groups' compliance rate was significantly higher than the control group.4.After intervention,influencing factors of lifestyle and KAP about blood lipid of the intervention group were better than the control group.Follow-up times of the intervention group was significantly more than the control group during the intervention period.5.Elevated levels of SBP,smoking increases,reduce the intake of vegetables,fruits,combined hypertension,diabetes,metabolic syndrome,and atherosclerosis other diseases,had not started lipid control,less lipid tests during the intervention period were risk factors of blood lipid indicators' compliance rate of dyslipidemia patients.Conclusion1.Community grading management of dyslipidemia can effectively reduce the patients' TC,TG,LDL-C levels.2.The effect of community grading management of dyslipidemia made no signaficate difference with patients' HDL-C level.3.Community grading management of dyslipidemia can effectively improve the compliance rate of blood lipid control.4.Elevated levels of SBP,smoking increases,reduce the intake of vegetables,combined hypertension,diabetes,metabolic syndrome,and atherosclerosis other diseases,had not started lipid control,less lipid tests during the intervention period were risk factors of blood lipid indicators' compliance rate of dyslipidemia patients.5.Community grading management of dyslipidemia in China needs to be further improved.
