Objective:to evaluate the clinical efficacy of the integrated scheme of traditional Chinese medicine and intrapleural perfusion in the treatment of malignant pleural effusion; to observe the adverse reactions of the integrated scheme of traditional Chinese medicine and intrapleural perfusion in the treatment of malignant pleural effusion; to explore the advantage of the Comprehensive protocol of integrated Chinese and western medicine in the improving patients' quality of life and reducing the adverse reactions of chemotherapy. Methods:Under the guidance of mentor, the integrated scheme of traditional Chinese medicine and cisplatin intrapleural perfusion in the treatment of malignant pleural effusion was performed from April 2014 to April 2015. The 54 patients with malignant pleural effusion were randomly divideded into treating group(27 patients) and control group(27 patients). The 27 patients of control group were in the treatment of intrathoracic cisplatin infusion therapy through the indwelling central venous catheters, once a week,4 times totally. The 27 patients of treating group were also in the treatment of intrathoracic cisplatin infusion therapy through the indwelling central venous catheters, once a week,4 times totally, and additionally, who diagnosed with the excess syndrome of pathogenic qi committing lung were given the Supplemented formule of Xiaochaihu decoction and with the deficiency syndrome of Yang deficiency water stop were given the Supplemented formule of Xiaochaihu and Guizhi Renshen decoction, a dose per day,28 dose totally. Observing the adverse reactions, short-term effects of malignant pleural effusion, improvement of life quality, efficacy of TCM Syndrome and so on, evaluate the clinical efficacy of the integrated scheme of traditional Chinese medicine and intrapleural perfusion in the treatment of malignant pleural effusion.Result: ?There was no statistically significant difference in age, gender, primary tumor, the volume of malignant effusions, Kanofsky score, TCM Syndrome score, TCM Syndrome type, blood routine, liver and kidney function between two groups(P>0.05). ?Short-term efficacy:Effective rate in the treating group was 73.08% and the control group was 68.00%. Remission-stability rate in the treating group was 96.15% and the control group was 76.00%. There was statistically significant different in effective rate and remission rate between two groups(P<0.05). ?Improvement of life quality:Clinical benefit rate in the treating group was88.46% and the control group was 68.00%. There was statistically significant different in Clinical benefit rate between two groups(P<0.05), which indicated treating group was superior to control group.?Kanofsky score:The Kanofsky score after treatment of treating group was 74.23±8.57 and control group was 67.20±8.91.There was statistically significant different in Kanofsky score between two groups(P<0.05).? Adverse reactions:There were no serious adverse reaction that stopped the treatment, such as heart and lung toxic, myelosuppression, liver and kidney function harm, which showed that integrated scheme of traditional Chinese medicine and intrapleural perfusion was safe. The negative impact by the treatment of local chemotherapy of white blood cells, serum creatinine and alanine aminotransferase of treating group was less than the control group according to the statistical data(p<0.05),which indicated Chinese medicine could alleviate the adverse reactions of pleural perfusion chemotherapy. Conclusion:Treatment effectiveness of Integrated scheme of traditional Chinese medicine and intrapleural perfusion was similar to simple intrapleural perfusion in the treatment of malignant pleural effusion. But the former was superior to the latter in terms of the remission-stability rate, improving clinical syndrome and life quality, alleviating adverse reactions of pleural perfusion chemotherapy, which could be promoted in the clinical.
