Chinese Medicine Combined With Western Medicine For Treating Deep Venous Thrombosis

Objective To explore the clinical characteristics and risk factors of the deep venous thrombosis(DVT),and to evaluate the clinical effects of pure Western medicine treatment and combination of Chinese and Western medicine treatment on acute DVT,and to observe the therapeutic effects of combination of Chinese and Western medicine treatment on acute DVT as well.Methods96 cases of deep vein thrombosis(DVT,in the acute period)patients,admitted by the Fist Affiliated Hospital of Xinxiang Medical University from August 2014 to August 2015 were took as test subjects.The incidence time of all the patients were all less than 72 hours.The medical records of those patients were completed.In accordance with the random number table,the 96 patients were divided into treatment group,control group1 and control group2,with 32 cases in each group.The three groups were all dealt with groovy processing measures,like bed rest,affected side elevation and so on.Then the treatment group were initiated pure thrombolytic therapy and anti-coagulation therapy.In the thrombolytic therapy,patients were placed 100~150 Uurokinase and 250 ml 0.9% sodium chloride by dorsalis drip phleboclysis 2 times per day with a course of 14 days.In the anti-coagulation therapy,patients were given 100 U/kg low molecular weight heparin by subcutaneous injection,1times/12 h,for 5 consecutive days.After 3 days of injections,patients were placed 2.5 mg oral Warfarin tablets for 30 days.On the basis of treating method of the treatment group,control group1 were given internal treatment of Chinese medicine as combination therapies and the oral Warfarin tablets were reduced to 5days.After 2 days stop of Warfarin,control group 1 were given Danqi Huoxue Tongluo Decoction(HTD),one dose a day for 30 days,200 ml each time,200 mlin the morning and200 ml in the evening.On the basis of treating method of the control group1,control group2 were given Xuangui Yinfu Tongluo powder by hot packs,boiling the chinese medicine package for 1 hour,then apply it to patient's injured limbs when the temperature reduce to 38?,two times one day,half an hour one time.The rate of DVT risk factors of all the patients was collected.The therapeutic effects of the three groups were compared.The swelling of injured thigh and calve before and after treatment,the establishment of injured limbs ‘s vessel after treatment,the stable value of International Normalized Ratio during the treatment and the incidence of adverse reactions were analysised.By following the three groups of patients from 12 to 24 month,the incidence of post-thrombosis syndrome was compared.All the data were analyzed by the SPSS19.0 software of statistics.Results(1)Patients with evident risk factors were 88.5%.Most of them were more then 40 years old,equivalent to 76.0%.There were 11 patients with unprovoked factor,about11.5%.(2)The clinical effect of the three groups were all significant.However the the total effective rate of observation group1(87.5%)and 2(93.8%)was significantly higher than the control group(P<0.05).There was no statistical significance between observation group1 and 2.(3)There was no statistical significance between the circumference of both thigh and calve of the 3 groups before treatment(P>0.05);The circumference of both thigh and calve after treatment of the 3 groups were significantly smaller than before the treatment(P<0.05);The circumference of both thigh and calve after treatment of observation group1 and 2 were significantly smaller than the control group(P<0.05).There was no statistical significance between observation group1 and 2(P>0.05).(4)The rate of completely establishment or half establishment of injured limbs' s vessel after treatment of observation group1 and 2 was significantly higher than the control group(P<0.05).There was no statistical significance between observation group1 and 2(P>0.05).(5)The stable value of INR of observation group1 and 2 was significantly higher than the control group(P<0.05).There was no statistical significance between observation group1 and 2(P>0.05).(6)The incidence of nose bleeds,subcutaneous haemorrhage and hematochezia of observation group1 and 2 during treatment was significantly lower than the control group(P<0.05).There was no statistical significance between observation group1 and 2(P>0.05).(7)The group1 and 2 ‘s incidence of PTS was significantly lower than the control group during follow-up(P<0.05).There was no statistical significance among the incidence of pulmonary embolism of 3 groups(P<0.05).There was no statistical significance between observation group1 and 2 too(P>0.05).Conclusion The method of pure western anticoagulant therapy combined with thrombolysis and the method of combining the above treatment mentioned and internal treatment of Chinese medicine both have a certain effect on DVT.However,the method of combined chinese-western treatment is superior to pure western treatment in clinical effect,improving the swelling of thigh and calve,reestablishment of injured limbs' s vessel after treatment and the stable value of INR during treatment.The combined chinese-western treatment can reduce the incidence of bleed and PTS during follow-up.
