Abstract/Summary:
BackgroundDiabetes is a serious public health problem worldwide.Currently,there are 382 million persons were diagnosed with diabetes according to the International Diabetes Federation.It is estimated that the diabetes mellitus(DM)patients will reach to 471 million.With economic growth rapidly,the prevalence of diabetes has increased quickly in China.The epidemiologic study indicated that there were 100 million DM patients in China in 2013(90% of the patients belong to type 2 diabetes mellitus).Prediabetes is a reversible process of DM and acted as an important role in DM prevention and control.Effective intervention of pre-diabetes could significantly reduce reduce the development of diabetic.With rapid urbanization process in China,a large number of landless peasants appeared in the last decades.Until 2016,the total number of landless peasants will be close to 100 million.Therefore,it is necessary to analyze the healthy conditions of the landless peasants.However,seldom epidemiological studied were performed to investigate the the prevalence of diabetes and prediabetes in landless peasants in China.Therefore,we carried out the present study to analyze the epidemiological characteristics of diabetes and prediabetes in landless peasants in the Suzhou Industrial Park.ObjectiveTo analyze the prevalence and the risk factors of diabetes mellitus and prediabetes in landless peasants,as compared with those of the urban residents in Suzhou.The results should be helpful for the further prevention and control of diabetes and prediabetes.MethodsThe participants included in the present study were selected by using cluster random sampling method.Briefly,4 communities were randomly selected from 4 township streets in Suzhou Industrial Park,respectively.Then,8,000 landless peasants aged more than 35 years were randomly selected in these communities.In addition,6548 urban residents lived in Suzhou(aged more than 35 years)were randomly selected as control in the present study.All investigators who participated in the present study were trained before survey.A face-to-face interview was carried out with all participants by a trained interviewer for the information about demographics(including age,gender,body weight,height and etc),Statistical analysis was carried out by SPSS22.0 after data processing.All reported P values are two-tailed,and those <0.05 were considered statistically significant.Results1)The prevalence of diabetes and prediabetes in landless farmers was 9.8% and 8.6%,respectively.The prevalence of diabetes and prediabetes in males and females was 9.6%,8.4% and 9.5%,8.8% respectively.The prevalence rate of diabetes and diabetes was 12% and 9.4% in urban residents.The prevalence rate of of diabetes and prediabetes in males and females was 12.5%,8.1% and 11.7%,10.4%.There was a significant difference between the prevalence of diabetes in urban residents and the prevalence of landless peasants(c2=18.074,P<0.01),as well as prediabetes(c2=10.425,P=0.001).2)The results showed that the prevalence of diabetes was higher in the 35-44 age group in landless farmers.However,the prevalence of diabetes mellitus in urban residents increased with age.3)Hypertension was significantly associated with the prevalence of diabetes in landless farmers and urban residents.Compared to urban residents,there was an increased risk of diabetes mellitus in the landless farmers with hypertension.In addition,hypertension was significantly associated with prediabetes among urban residents(P=0.002).However,there was no significant correlation between hypertension and the early stage of diabetes.In addition,the prevalence of diabetes and prediabetes were significantly higher in overweight and obese individulas,regardless of landless peasants or urban residents(P<0.01).4)There was a relationships betwteen drinking and diabetes in landless farmers(P=0.045).Additionally,smoking was significanltly associated with the prevalence of prediabetes in the landless farmers(P=0.012).However,there was no relationship between smoking,alcohol consumption and diabetes,early stage of diabetes in urban residents.Conclusion 1)The prevalence of diabetes mellitus and prediabetes mellitus in landless farmers was significantly lower than those in urban residents.Compared to urban residents,drinking and smoking were significantly associated with diabetes and prediabetes in landless farmers,respectively.2)Hypertension,overweight and obesity were significantly associated with the prevalence of diabetes among the landless peasants and urban residents.
