Objective To investigate the relationship between Glu CV and other clinical indices, and the prognostic value in diabetic patients with sepsis. Methods A total of 264 diabetic patients with sepsis admitted in the department of emergency medicine of Tianjin Medical University from June 2013 to June 2015 were divided into survival group and nonsurvival group. A variety of clinical and laboratory variables were recorded including Age, CRP, TG, TC, Glu CV, APACHE II and other indices. According to the level of Glu CV, patients were divided into 4 groups, CV1<0.21; 0.21? CV2<0.23; 0.23?CV3<0.35;CV4?0.35 to compare CRP, TG, TC, HDL, LDL, Glu CV, APACHE II, mortality and the times of hypoglycaemia of the four groups. ROC was draw and the area under ROC curve was calculated. Statistical analysis:SPSS 19.0 software was used for statistical analysis. The data were performed K-S normal distribution test. Normal distribution of measurement data as mean ± standard deviation( x ±s). Two independent samples use T test, and four groups were compared using One-way ANOVA analysis of variance. Pairwise comparisons using LSD test. The non-normal distribution of the data using the median(quartile range), and the Mann-Whitney U test.Classified information use the ratio of number and using ?2 test. Using Spearman correlation to analyse the relationship between different levels of Glu CV and prognosis. ROC curves were calculated to compare the size of the area under the curve. P<0.05 was considered statistically significance. Results 1.Nonsurvival group and survival group CRP(15.07±11.66 vs 8.70±6.82); Glu CV(0.46±0.06vs0.22±0.03),P<0.05;TG(1.78±0.52vs1.48±0.51),P<0.05;APACHE II(18.93±6.54vs12.48±5.58),P<0.05;the rate of hypoglycaemia(0.422 vs 0.095, P<0.05); and Age( 71.75±8.11vs66.6±12.75), P>0.05; TC(4.97±0.91vs4.73±0.69),P>0.05. 2.(1)The value of CRP:group CV1(8.61±6.70)and group CV4(15.07±11.66),P<0.05;group CV3(8.10±5.02)and group CV4 ?(15.07±11.66),P<0.05.(2)The value of TC:group CV3(4.40±0.75)and group CV4(4.97±0.91),P<0.05.(3)The value of TG:group CV3(1.33±0.61)and group CV4(1.78±0.52),P<0.05.(4)The value of LDL:group CV1(2.41±0.82)and group CV3(3.06±0.46),P<0.05;group CV2(2.93±0.81)and group CV3(3.06±0.46),P<0.05;group CV3(3.06±0.46)and group CV4(3.31±0.60),P<0.05.(5)The value of HDL:group CV2(0.94±0.41)and group CV3(1.24±0.65),P<0.05;group CV3(1.24±0.65)and group CV4(0.77±0.21),P<0.05.(6)The value of APACHE II:group CV1(10.50±5.88)and group CV4(18.94±6.54),P<0.05;group CV2(12.47±6.77)and group CV4(18.94±6.54),P<0.05;group CV3(13.47±3.50)and group CV4(18.94±6.54),P<0.05.(7)The rate of morality:group CV1 and CV2(0.015vs0.147),P<0.05;group CV1 and CV3(0.015vs0.422),P<0.05;group CV1 and CV4(0.015vs0.766),P<0.05;group CV2 and CV3(0.147vs0.422),P<0.05;group CV3 and CV4(0.422vs0.766),P<0.05.(8)The rate of hypoglycemia:group CV1 and CV3(0.074vs0.219),P<0.05;group CV1 and CV4(0.074vs0.297),P<0.05;group CV2 and CV3(0.118vs0.219),P<0.05;group CV2 and CV4(0.118vs0.297),P<0.05.(9)The value of age and length of hospital stay,P>0.05. 3. There were positively correlated between CRP,TG,APACHE II and Glu CV. r=0.220?0.133?0.201; Glu CV had no correlation with Age,TC,LDL,HDL(P>0.05). 4. The AUC of Glu CV,CRP,TG,APACHE II were 0.856?0.613?0.674?0.752. 5.The AUC of Glu CV in different levels were 0.796?0.916?0.824?0.895.Conclusion1.GluCV,CRP,TG,APACHE II were indicators to determine the prognosis of diabetic patients with sepsis. 2. There were positively correlated between CRP,TG,APACHE II and Glu CV, The higher the Glu CV level was, the higher CRP,TG and APACHE II was. 3.Patients of Glu CV(?0.35)were in higher CRP?APACHE II and the rate of hypoglycemia than other groups. And Glu CV(?0.35)level had a certain significance in judging the prognosis of diabetic patients with sepsis.
