BackgroundDiabetes Mellitus(DM) was characterized by the glycemia disorder, which included the chornic sustained hyperglycemia and acute glucose fluctuation, more and more researches represent excessive glucose fluctuation have the more important meaning in diabetic chronic complications. Many researcheres conclude when we control the glucose, we must not only think about the measure: fasting and postprandial glucose, HbA1c, but also the quality: glucose fluctuation. Only when we take attention to both of the two sides, can we correct the metabolic disturbance of diabetic patients, and prolong the complications'happen and develop.ObjectiveTo compare the different monitoring indexes of blood glucose fluctuation between the vasculopathy group and the no vasculopathy group , and discuss the practical applications in diabetic blood glucose monitoring between CGMS and SMBG.Methods1.Select thirty type 2 diabetic in-patients who used the continuous glucose monitoring system(CGMS) , they were in hospital no less than fourteen days and monitored blood glucose no less than four times per day. These patients had been divided into two groups:the vasculopathy group, and no vasculopathy group according to the medical history such as coronary heart disease, stroke and so on, the 24 hours microalbuminuira(MAU), eyeground examination, and type-B ultrasonic of artery.2. The clinical data had been compared between them: age, course of disease, HbA1c, BMI, WHR, TC, TG, HDL-C, LDL-C, SBP, DBP, SD, MAGE, ADRR, MBG, LAGE, CV-FPG, PPGE. To use Pearson analysis analyse the relationship between MAGE and ADRR, PPGE, respectively.ResultsTo compare the two groups, there were significant difference in age, course of disease, BMI, FBG, P2hBG, blood presure, MAGE, ADRR, MBG, PPGE(P<0.05), and there were no significant difference in blood fat, SD, LAGE, and HbA1c, CV-FPG. Pearson correlation analysis show ADRR and PPGE were associated with MAGE(r=0.550, P<0.05 and r=0.508, P<0.05,respectively).ConclusionsDiabetic vasculopathy group had a larger fluctuation of blood glucose than no vasculopathy group, it suggested that blood glucose fluctuation had a significant meaning in the development of diabetic complication, and compare the different indexes of blood glucose fluctuation between CGMS and SMBG, the indexes of SMBG can also reflect blood glucose fluctuation.
