Objective To investigate the self-nursing behavior level, psychological status and social adaptation ability owned by discharged digital replantation patients, and we want to understand the influence factors of functional recovery and social adaptation ability for them. To provide evidence of the self-care mode for postoperative patient and make them the maximum recovery function and return to society.Contents All cases of test group received synthetic self-nursing behavior(Orem self-care model) interference, while the control group cases was completed by conventional health education method only. Both groups accepted self-nursing behavior level,mental status and social adaptation ability investigation via self-made Digital Replantation Patients Self-care Behavior Scale(DRPSBS) and Texas Social Behavior Inventory(TSBI) after discharge. Both received comprehensive test and evaluation through using above questionnaires in order to evaluate the effects of Orem self-care model on the self-nursing ability, psychological state and social adaptation ability in 1month and 3 months after discharge.Methods1 Establishing the evaluation index:Designing the questionnaire: After interviewing twenty cases patients who received a operation of digital replantation, and we referred to Orem self-care theory and related literature. We established four dimensions and twenty entries of the "replantation patients self-care behavior questionnaire" by using the Delphi method to score questionnaire every objective importance, which was performed by six experts,and carry on preliminary experiment, analysis of reliability and validity of the questionnaire. Using the TSBI to evaluate the psychological and social adaptation ability of patients.2 Study and establish comprehensive Orem self-care mode of the patients who received a operation of digital replantation.One hundred and three adult patients of a grade A class 3 hospital were used as subjects according to the inclusive and exclusive criteria defined before. They were randomly divided into two different groups according to their registration numbers:fifty-two patients with odd numbers as experimental group and the other fifty-one with even number as control group. The experimental group accepted comprehensive self-care behavior intervention and the control group accepted routine health education methods.4 Analysing data:Investigate self-care behavior level and psychological and social adaptation ability of patients after discharge by using Digital Replantation Patients Self-care Behavior Scale(DRPSBS) and Texas Social Behavior Inventory(TSBI). Both received comprehensive test and evaluation through using above questionnaires in order to evaluate the effects of Orem self-care model on the self-nursing ability,psychological state and social adaptation ability in 1 month and 3 months after discharge.We organized and statistical analysis by using SPSS16.05 Revise and improve comprehensive self-care behavior in patients who received an operation of digital replantation according to the statistical results.3 Establishing the study objects:Result1 Through in-depth interviews, this study refer to relevant data, expert evaluation,finally made the patients self-care of the wounded finger behavior questionnaire.2 The comparison of self-nursing behavior level between the experimental group and the control group showed statistical difference(P<0.05) : the two groups had statistical difference in control behavior and inspecting behavior dimensions(P<0.05)1 month after discharging from the hospital, but had no statistical difference in daily behavior and processing behavior dimensions(P>0.05). However, the investigation statistic data showed that test group patients got advantage over the control group in all of the four dimensions 3 months after discharging from hospital, that is to say the difference had statistical significance. From the statistic data we can see that the advantage of Orem Self-care Model were standing out along with time.3 The investigation statistic data of 1 months as well as 3 months after discharging from hospital showed that test group patients had advantage over the control group in mental and social ability(P<0.05), and the dominance reinforced gradually along with time. We can concluded from the radar distribution of TSBI experimental group had markedly advantage. Experimental group patients in seven aspects such as the active exchanges and speak more advantage.4 Experimental self-care behavior and psychological social adaptation ability had no difference in patients with different ages and sex. The patients with college degree or above received a higher control and processing capacity and psychological social adaptation ability than the patients with a low diploma.Conclusion1 Through the research to argue that the self-care behavior in patients with the questionnaire of the wounded finger in the assessment of the patient's self-care of the wounded finger behavior ability, has the high scientific and use value.2 Orem self-care behavior model arouse patients' self-nursing capacity, and help to furthest restore the function of injury fingers.3 Orem self-care behavior model can enhance patients' confidence and improve their social adaptation ability.
