Changes And Its Clinical Significance Of The Plasma Hydrogen Sulfide And Matrix Metalloproteinase-2 In Patients With Chronic Congestive Heart Failure

Objective To investigate the changes and clinical significance of the plasma hydrogen sulfide(H2S), matrix metalloproteinase-2(MMP-2) in patients with chronic congestive heart failure(CHF).Methods Select 100 cases CHF patients in cardiovascular medicine department of ningxia medical university cardio-cerebrovascular disease hospital from October 2013 to April 2014. Patients were divided into three groups according to the New York Heart Disease Association(NYHA) heart function classification standard, including NYHAⅡ grade group(n=35), NYHA Ⅲgrade group(n=32) and NYHA Ⅲgrade group(n=33). CHF patients were also divided into left ventricular ejection fraction(LVEF)<40% grade HFr EF group(n=52) and LVEF≥40% grade HFp EF group(n=48) in according to LVEF. 30 healthy persons were selected as control group. Extract venous blood from selected objects. Venous blood was used to detect H2 S, MMP-2 and BNP by ELISA method. At the same time, Venous blood was used in the detection of blood routine and biochemical indexes. LVEF, LVDd, LVDs, LVFS, IVST, PWT, LVM, LVMI detection were detected by echocardiography.Results 1. Compared with control group, the levels of H2 S were significantly lower in NYHAⅡ, Ⅲ, Ⅳ group(76.01±22.13; 65.71±16.29; 51.72±15.65 VS 86.82±24.78) and significantly lower in HFp EF group and HFr EF group(72.19±20.82; 57.78±18.38 VS 86.82±24.78)(P<0.05). There were negative correlation between the plasma level of H2 S and Lg BNP, NYHA, LVDd, LVDs, LVM and LVMI(r=-0.749,-0.544,-0.361,-0.388,-0.292 and-0.322, P<0.05). The plasma level of H2 S was positively correlated with LVEF and LVFS(r =0.429, 0.412, P<0.05).2. Compared with control group, the levels of MMP-2 were significantly higher in NYHA Ⅱ, Ⅲ, Ⅳ group(2.64 ± 0.64; 3.00 ± 0.62; 3.16 ± 0.68 VS 2.13 ± 0.52) and significantly higher in HFp EF group and HFr EF group(2.78±0.71; 3.07±0.62 VS 2.13±0.52)(P<0.05). NYHAⅢgrade and Ⅳgrade were higher than NYHAⅡ(P<0.05). There was no difference between NYHA Ⅲ grade and Ⅳ grade(P>0.05). There were positive correlation between the plasma level of MMP-2 and Lg BNP, NYHA, LVDd, LVDs, LVM and LVMI(r=0.469, 0.523, 0.385, 0.415, 0.345 and 0.367, P<0.05). The plasma level of H2 S was negatively correlated with LVEF and LVFS(r=-0.462,-0.454, P<0.05).Conclusions 1. The levels of H2 S were significantly lower in heart failure group with NYHA class increasing. H2 S was negatively correlated with BNP and positively correlated with LVEF.2. H2 S was negatively correlated with LVDd, LVDs, LVM and LVMI.3. The levels of MMP-2 were significantly higher in heart failure group with NYHA class increasing. MMP-2 was negatively correlated with LVEF and positively correlated with BNP.4. MMP-2 was positively correlated with LVDd, LVDs, LVM and LVMI.
