Objective: The aim of this study is to observe the vital signs, inflammatory reaction parameters, coagulation, APACHE II score, clinical symptoms score, gastrointestinal dysfunction of the patients with sepsis, and 14 and 28 natural survival rate as evaluation indexes of clinical curative effect. To observation an optimized regimen for the treatment of sepsis patient with sepsis clinical curative effect, for the further optimization of the scheme of treatment of sepsis provide useful help.Methods: Ospective, randomized, double-blinded clinical trial. Patients who had been selected were randomly divided into two groups: treatment group and control group. Each group had 25 Patients, the total was 50. Both of them were given conventional treatment and Xuebijing Injection. In addition, the treatment group received Lianggesan granules, 150 ml twice daily, for 7 days. The control group received a placebo treatment. The medicines can be taken through mouth or through a gastric tube. Respectively at the 1th day, the 3th day, the 7thday, the following examination and experimental indexes of the patients were observed: the related symptoms and signs; to calculate the score about the gastrointestinal dysfunction according to 95 Lushan meeting criteria, APACHE II score, DIC score; detecting blood routine, blood gas analysis, blood coagulation full term, CRP, PCT, TNF-a, IL-1?, IL-10; to write down the mortality within 28 days.Results:1 In the indicators of inflammation, the temperature of the treatment group decreased more significantly than the control group, the difference is statistically significant(P<0.05), PCT, WBC, TNF-a levels of treatment group compared with the control group decreased significantly(P<0.05), while the IL-10 of the treatment group increased significantly compared with those before treatment(P<0.05).2 In terms of coagulation function, APTT of reatment group didn't significantly prolonged, as compared with before treatment was significantly longer than control group on the seventh day of treatment, the difference has statistical significance(P<0.05); D-dimer level of treatment group was significantly decreased than the control group on the third day of treatment, the difference is statistically significant(P<0.05). On the seventh day, PLT of the treatment group was significantly higher compared with the control group, the difference was statistically significant(P<0.05). Seventh days, the DIC score of treatment group was lower than that before treatment, the difference was statistically significant compared with the control group(P<0.05).3 The APECHE-II score of control group and treatment group was decreased than before treatment; On the seventh day, the APECHE-II score of treatment group was decreased significantly than before treatment, the difference was statistically significant(P<0.05); In third days and seventh days, the clinical symptoms scores of two groups were significantly decreased compared with before treatment(P<0.01), compared with the control group, the treatment group was significantly lower than the control group, the difference was statistically significant(P<0.01). The comprehensive efficacy of the two groups, the control group was 20%, the efficiency of the treatment group was 60%, the treatment group was significantly higher than the control group, the difference was statistically significant(P<0.05).4 In the control group and treatment group, the fourteenth day mortality rates were 24% and 16% respectively, the twenty-eighth day mortality rate was 32% and 24% respectively, and there was no significant difference between the two groups(P>0.05).Conclusion:Treatment plan of Jun Du Yan Bing Zhi can effectively decreased the body temperature and WBC, PCT, CRP level of patients with sepsis, can better reduce the concentration of TNF-a in the patient's body, inhibit inflammatory reaction from the source. Can promote the release of IL-10, inhibit the production of inflammatory mediators, and thus to regulate the dynamic balance of pro-inflammatory and anti-inflammatory responses, contribute to the control of systemic inflammatory response. The treatment can improve the coagulation function index, improve blood coagulation dysfunction, improve the prognosis of patients to a certain extent, it is worthy of clinical application.
