Clinical Effect Observation Of Xuebijing Injection Combined With Ulinastatin In The Treatment Of Sepsis

Objective: Based on the clinical data of 88 cases of patients with sepsis met the inclusion criteria from First Affiliated Hospital of Dalian Medical University from January 2010 to November 2015 were retrospectively analyzed,and evaluated clinical efficacy of sepsis by Xuebijing injection combined with Ulinastatin to provide experience for the clinical treatment of sepsis.Methods:According to "Chinese severe sepsis / septic shock treatment guidelines(2014)" formulated by the Critical Care Medicine Branch of Chinese Medical Association provides the diagnosis of sepsis,severe sepsis,septic shock criteria,select the First Affiliated Hospital of Dalian Medical University from January 2010 to November 2015 in line with the inclusion criteria of clinical data 88 cases of patients with sepsis to retrospective analysis.Access to clinical data,according to the drug(Xuebijing injection and/or UTI)treatment usage,divide Conventional treatment group(conventional therapy,27 cases),Xuebijing treatment group(conventional therapy +Xuebijing injection,29 cases)and the Combined drug group(conventional therapy +Xuebijing injection+ Ulinastatin,32 cases).Record three groups of patients in general gender,age,medical history,the site of infection,drugs use,etc.A detailed record of three groups of patients before treatment and after treatment for 7 days in patients with body temperature(?C),peripheral blood white blood cell(WBC)count,the neutral granular cell proportion(NEUT),lactic acid(LAC)concentration,platelet count(PLT),and clotting four [activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(Fib)],organize the relevant organ function parameters [blood creatinine(CRE),blood urea nitrogen(BUN),total bilirubin(TBil),ultra sensitive troponin I(hs-Tn I)] and other indicators of changes.According to the symptoms,signs,laboratory examination(peripheral blood white blood cell count),clinical curative effect is divided into healing,effective,progress,Invalid.Markedly effective rate = [(number of cured patients + number of effective cases)÷ total number of cases] × 100%.Results: 1.88 cases of sepsis patients,male 50 cases(57%),38 females(43%);the age of men was 52.80±14.27 and women was 54.03±12.79.Sepsis 46(52%)patients,33(38%)patients with severe sepsis,and septic shock 9(10%).Infection site: lung was 39 cases and urinary tract 19 cases,abdomen 13 cases,skin 9 cases,6 patients with pancreatic,the other 11 cases;2.Three groups of patients before treatment,the differences were not statistically significant(P>0.05);the temperature,WBC,NEUT,Lac,APTT,PT,TT,CRE,BUN,,hs-Tn I,TBil,were significantly decreased compared with before treatment,Fib,PLT were significantly higher than before treatment,and the Combined drug group was more significant changes after treatment(P<0.05);3.Xuebijing treatment group compared with the Conventional treatment group of indicators after treatment: temperature,WBC,Lac,PT,TT,CRE,BUN,TBil of Xuebijing treatment group decreased significantly compared to the Conventional treatment group(P <0.05),PLT compared with the Conventional treatment group was significantly higher(P < 0.05);4.Combined drug group compared with the Conventional treatment group of indicators after treatment:temperature,WBC,NEUT,Lac,APTT,PT,TT,CRE,BUN,TBil,hs-Tn I of combined drug group decreased significantly compared to the conventional treatment group(P <0.05),PLT,Fib significantly increased(P <0.05)compared to the the Conventional treatment group;5.Combined drug group compared with the Xuebijing treatment group ofindicators after treatment: WBC,NEUT,APTT,PT,CRE,TBil in Combined treatment group decreased significantly compared to Xuebijing treatment group the same period(P <0.05),Fib compared to the Xuebijing treatment group increased significantly(P < 0.05);6.Markedly effective rate of the Conventional treatment group,Xuebijing treatment group and Combined drug group is 74.1%,82.8%,87.5%.Conclusion: On the basis of routine treatment on Xuebijing,UTI can synergistically inhibit inflammation in septic patients,improve the body's blood coagulation disorders and microcirculation,improve the clinical symptoms and the corresponding index,protect the heart,liver,kidney and other vital organ function,thus effectively improve markedly effective rate of clinical treatment.
