Objective: To study the clinical features and surgical complications after operative treatment for gastric carcinoma in the patients older than 65 years. then to introduce some treatment experience for the elderly gastric carcinoma patient.Methods: From January 2005 to January 2007, 106 gastric carcinoma cases were treated by my medical group, out of them,43 cases were older than 65 years, the age related preoperative complications(i.e. hypertension, coronary artery disease, EKG change, diabetes mellitus, respiratory diseases, liver function insufficiency, liver cirrhosis, hypoalbuminemia, anaemia, cerebrovascular diseases, electrolyte imbalance etc.) and postoperative complications(incision infection, chest infection, anastomotic fistula, intra abdominal infection, anastomotic bleed, intestinal obstruction, intra abdominal gross hemorrhage, lymphatic fistula, arrhythmia, liver dysfunction etc.) were analyzed.Result: The incidence of preoperative complications on the elderly gastric carcinoma patient is 79.1%(34/43), compared with normal group is 47.6%(30/63); About 51.2%(22/43) cases presented with two or more than two complications. Among them the incidence of hypertension is the highest is 39.5%(17/49), the radical curative rate of the elderly group is 86.0%(37/43) as compared with normal group is 88.9%(56/63). The postoperative complications noticed in elderly group is 37.2%(16/43). Among the elderly cases who have postoperative complications, the incidence of preoperative complications (such as hypertension, diabetes, respiratory diseases, anaemia, hypoalbuminemia)is relatively higher in the patients who have no complications.Conclusion: The age is not a single influential factors so far as postoperative complications, morbidity and mortality is concerned. Moreover, several preoperative complications are independently related with postoperative complications (hypertension, diabetes, respiratory diseases, hypoalbuminemia, anaemia)and mortality.
