OBJECTIVES: We aimed to find out the natural course and prognostic factors of early-onset AD in Chinese Han population.METHODS: A total of 450 early-onset AD(onset in the first 2 years), whose genotype of FLG have previously been clarified by comprehensive sequencing in 2008 and 2009 were recruited. The remission rates among various age groups and patients with different FLG mutations were compared using chi-square test. Prognostic factors were analyzed by Cox proportional hazard model.RESULTS: Ultimately, 284 patients were successfully followed up. Fifty-three(34.6%)out of 153 infants(<2 years at baseline) went to disease remission by age 3, while 103 of them(67.3%) went to remission by age 5. Moreover, the 5-year remission rate decreased obviously with the increasing of age at baseline. By age five 35.9% of the patients gave a history of asthma and 49.7% gave a history of rhinitis. The incidence of asthma and rhinitis were increased by 24.8% and 20.3%, respectively. The male gender(P=0.032),FLG mutation(P=0.021) and SCORAD index(P=0.041) were associated with the prognosis of AD. Presence of respiratory atopy(rhinitis or asthma) presence of family atopy(rhinitis, asthma or AD) did not show statistically significant influence on disease course. And different FLG mutations did not show statistically significant influence on the remission of AD.CONCLUSIONS: In China, by age 3 and 5 the remission rate of AD is 34.6% and 67.3%,respectively. Male gender, high SCORAD and FLG mutation were risk factors for non-remission AD. The presence of AR or asthma or family atopy did not affect the disease course of AD. And the different FLG mutations did not contribute the remission of AD.
