Evaluation Of Medium Low-dose Prophylactic Treatment Of Coagulation Factor ? On Hemophilia A Patients At Guangzhou

Research background and goalsThe goal of this thesis study is to analyze whether the hemophilia population after prophylactic treatment has reached expected life quality after the prevention treatment. The collected data mainly based on the two data sets from the “the basic expenses of the medium low-dose prophylactic treatment for Hemophilia type A patients " questionnaire and the " Prevention and treatment of injection Handbook”. These two groups of retrospectively analyzed data and the information on the prophylactic treatment of the hemophilia population are close related.After collecting the following two different sets of data:(1) “Basic expenses of the medium low-dose prophylactic treatment for Hemophilia type A patients ";(2) " Hemophilia prophylactic treatment injection Handbook", the Cost Utility Analysis(CUA) method was applied to study these two data sets. The results of this analysis have truly and directly represented the real condition of the life quality of hemophilia population after prophylactic treatment.Research MethodsThis study collected two retrospective data sets and utilized CUA method for data analysis.1. Using the survey of "Basic expenses of medium low-dose prophylactic treatment for Hemophilia type A patience" to mainly evaluate the gap of life quality between adults and children after prophylactic treatment.2. Using "Prophylactic treatment of hemophilia injection Handbook" to calculate weekly dose, yearly dose, weekly expense and yearly expense for medium-dose and low-dose prophylactic treatment. CUA method was applied to analyze two data sets which were from the surveys of "Hemophilia injection Handbook" and " Hemophilia type A patience of medium low-dose prophylaxis basic expenses". This evaluation is focus on the gap of medical expenses and life quality within hemophilia patient after the medium low-dose prophylactic treatment.Research Results1. Data analysis from the survey showed that children without health insurance are the most major issues for their life quality. However, the pediatric patients with preventive treatment showed no significant difference in the life quality compared to healthy children. The adult patients received preventive treatment showed significant improvement in life quality. However, the adult patients have the most major issues of 60% unemployment rate. Because of the high unemployment rate, the adult patients can't have the same normal life as the healthy adults. The adult patients showed obvious anxiety and depression in two kinds of psychological tests.2. Although annual dose usage and total annual cost for preventive treatment in medium low-dose are twice more than the preventive treatment in low-dose, due to the patients with the preventive treatment in medium low-dose have purchased Guangzhou Medical Insurance, the cost of the medication does not have much psychological and economic pressure on the patients. However, since these patients started the prophylactic treatment around 30-40 years old, there are four patients with mild anxiety and one patient with mild depression from the medium low-dose prophylactic treatment population in two kinds of psychological tests.ConclusionAdult and pediatric patients groups with prophylactic treatment has met the requirement of CUA method.1. Advantages of prophylactic treatment:(1) Since Guangzhou Hemophilia Medical Insurance has a reimbursement rate of 80% to 90% for adult and 70% for children(see Table 18), this reimbursement can cover the treatment cost for hemophilia patients;(2) This research data show that medium low-dose prophylactic treatment is better than the demand treatment. Compared to the demand treatment, the prophylactic treatment can not only reduce the number of bleeding and slow down joint deformed, but also be able to improve the operational capacity of the joints in hemophilia patients(see Table 48).(3) The Prophylactic treatment can achieve to reduce degree of mentality of fear and anxiety from the adult and pediatric hemophilia patient groups, to reduce degree of mental illness such as: anxiety and harassment of depression(see Table 49;50);(4) This data indicated that the recovery of the body's health skills allows patients to participate social work and social activities as soon as possible and to perform a normal healthy work without too reliance of accompany of the family members. This reduction of the family burden can enhance family happiness, make the society more stable, and create a better social and economic effects.2. Disadvantages of prophylactic treatment: The data showed(see Table 54; 56) that the medium low-dose prophylactic treatment for hemophilia patients has twice higher expenses than the demand treatment. For the patients without Guangzhou Hemophilia Medical Insurance, it will increase financial difficulties which need government or social strength, according to the data analysis from this article, to provide the population with reasonable cost support. That is another objective of this study.
