| ObjectiveTo explore the clinical characteristics,therapy and prognosis of thyroid carcinoma in elderly people,expect to improve the patients’ survival time and quality of life.MethodsA retrospective analysis in the First Affiliated Hospital of Fujian Medical University from January 2006 to December 2011, admitted to the surgical treatment of 122 cases of thyroid cancer in elderly. Randomly selected surgical treatment of 287 cases of thyroid cancer in non-elderly during the same period.Collect and analyze the cases of gender, pathological type, tumor size, multifocal, extrathyroidal invasion,lymph node metastasis, distant metastasis, clinical stage,underlying disease,surgical pattern and cervical lymph node dissection. Comparative analysis of clinical and pathological features of the two groups,calculate the overall survival, and analyze the impact of various factors on the prognosis of thyroid cancer in elderly.ResultsCompared with the control group found that : thyroid carcinoma in elderly people merge more underlying diseases,adverse histological type, tumor diameter larger, more multifocal, higher rate of violations outside the gland, low rate of lymph node metastasis, higher rate of distant metastasis, higher clinical stage, the difference was statistically significant. While sex was no significant difference between the two groups,and was not statistically significant.The 5 year cumulative survival rates was 87.7%(107/122). In the 15 cases of deaths, 12 cases had died of the Thyroid carcinoma with lung, bone and other metastasis,or due to tumor cachexia. 1 case died of lung cancer, 1 case died of renal cancer, 1 patient died of cerebral vascular accident.To the end of follow-up, the eight cases of new onset of distant metastasis, including lung metastasis in 4 cases, 2 cases of bone metastasis, lung, bone metastases in 2 cases.Univariate analysis showed that: pathological type(P = 0.013), tumor size(P = 0.002), clinical stage(P = 0.015), distant metastasis(P = 0.000)were significantly associated with the prognosis of thyroid cancer in elderly people. While gender, lymph node metastasis, extrathyroidal invasion, multifocal and cervical lymph node dissectionwere no significantly associated with prognosis(P> 0.05). Multivariate analysis showed that: distant metastasis(P = 0.005) were independent risk factors for thyroid cancer overall survival of the elderly.Conclusion1 Elderly Thyroid cancer compared to non-elderly, there are features include merge more underlying diseases,adverse histological type, tumor diameter larger, more multifocal, higher rate of violations outside the gland, low rate of lymph node metastasis, higher rate of distant metastasis, higher clinical stage.2 Pathological type, tumor size, clinical stage, metastasis were related to the prognosis of thyroid cancer in elderly people.And distant metastasis is an independent risk factor for thyroid cancer overall survival of the elderly.3 According to the patient’s condition, to assess the prognostic factors to take individualized treatment.In the effort to cure cancer combined with clinical and pathological characteristics of the elderly, to avoid over-treatment and reduce complications to improve quality of life and survival of patients. |
