| Part I:The analyse of the risk factors of hospital acquired pneumonia of the patients in department of neurology in a 3-A-grade hospital[Objective] To analysis independent risk factor of hospital acquired pneumonia of the patients in department of neurology in a 3-A-grade hospital. And understand the high-risk sectors and groups. To provide a scientific basis and recommendations for formulating and adopting corresponding prevention and control measure.[Methods] To investigate the patients in department of internal neurology and department of neurosurgery of a 3-A-grade hospital from January, 2013 to June, 2014,using the methods of retrospective and prospective investigation and the methods of a case-control study. To use SPSS16.0 software package for data entry and analysis. To analysis risk factors of hospital acquired pneumonia by using Logistic regression. P<0.05 was considered as statistically significant.[Results] The independent risk factors of hospital acquired pneumonia of the patients in department of neurology include disturbance of consciousness(OR=3.137), Icu admission(OR=2.985), dysphagia(OR=2.685), operation(OR=l.997),emergency admission(OR=l.697), diabetes mellitus(OR=l.436),age(OR=1.211)。The variables were positively correlated associated with hospital acquired pneumonia.[Conclusion]1.Seven exposure factors including disturbance of consciousness, ICU admission, dysphagia, operation,emergency admission, diabetes mellitus,age,which were the main risk factors 0f hospital acquired pneumonia of the patients in department of neurology.2. Hospital acquired pneumonia occurs is the result of the combined effects of multiple factors. We should develop effective measures against the risk factors and related links in the prevention and control.3. The key measures for the prevention and control of hospital acquired pneumonia include to protect vulnerable populations, to control blood sugar positively,to avoid the unnecessary invasive procedures, rational use of antibiotics and so on.Part II:The impact on the hospital costs and length of stay from the hospital acquired pneumonia of the patients in department of neurology[Objective] To investigate the impact of hospital acquired pneumonia for the hospital costs and the number of days of hospitalization of the inpatients in department of neurology in a 3-A-grade hospital.To evaluate the direct economic losses caused by its.[Methods] To collect the information of the patients in department of internal neurology and department of neurosurgery of a 3-A-grade hospital from January, 2013 to June, 2014, using the methods of cretrospective and prospective investigation and the methods of 1: 1 matched case-control study. To use SPSS16.0 software package for data entry and analysis.To analysis the impact on the hospital costs and length of stay from the hospital acquired pneumonia. P<0.05 was considered as statistically significant.[Results] Duing to the hospital acquired pneumonia,the medical costs increased by 22133.50 yuan. The major increase of medical expenses was drugs cost. The next were the cost for the examinations and testing. The length of stay in case group was longer than that in the control group for 9 days and the difference was statistically significant(P<0.05).[Conclusion] The occurrence of hospital acquired pneumonia will increase the cost of hospitalization, prolong the hospital stay,and then, cause the bed turnover rate reduced and bring a huge burden and pressure to community health resources. Although it is not possible to avoid from hospital acquired pneumonia completely, but the medical staff should actively take measures actively to have a good work for the prevention and control of hospital acquired pneumonia of the patients in department of neurology.So that let the limited health resources to maximize social benefit and economic benefit. |
