Objective: Through two-dimensional gel electrophoresis and mass spectrometry identification of MALDI-TOF-MS, previous studying team separated out 70 kinds of protein with differential expression from gastric cancer and adjacent normal mucosa tissue, in which has 43 kinds of protein with low expression in the gastric cancer tissue. This topic carried out studying about the AKR1C3 protein with the decreased expression in gastric cancer tissue, further verifying the low expression of AKR1C3 in gastric cancer tissue, analyzing the relationship between it and clinical pathological characteristics and prognosis, and judging its clinical value in the prognosis of gastric cancer.Method: To collect 8 pairs of fresh gastric cancer and adjacent normal mucosa tissue, and adopt method of Western blot to carry out quantitative determination for the expression situation of the AKR1C3. Collection of 137 cases of operative specimen of gastric cancer which admitted by Gastroenterology, the First Clinical Medical College from 2008 to 2010, making tissue microarray. To adopt the IHC to carry out location detection for the expression level of AKR1C3 in gastric cancer and adjacent normal mucosa tissue. Compare the relationship between the expression level of the different AKR1C3 and clinical pathological characteristics of gastric cancer, which including gender, age, degree of differentiation, tumor location, TNM-staging, invasive depth, lymph node metastasis, vessel carcinoma embolus, nerve invasion and size of the tumor. In combination with follow-up data, carry out the survival analysis of the patients with gastric cancer by adopting the method of Kaplan-Meir, and carry out analysis of the single factor and multiple factors of the gastric cancer prognosis by adopting the Cox proportional hazard model.Results: 1. The results of Western blot showed that protein expression of the AKR1C3 is lower than the normal tissue in chosen 7 pairs of gastric cancer tissue, in which the expression is high in the third pair, so initially determined that the AKR1C3 protein shows the low expression in gastric cancer tissue. 2. The results of immunohistochemistry of the gastric cancer tissue microarray showed that the expression of the AKR1C3 in gastric cancer tissue(24.8%) is significantly lower than the adjacent normal mucosa tissue(68.6%). The difference has the marked statistical significance( P<0.001), which is consistent with the results of the Western blot. 3. To verify and compare the relationship between the different expression level of AKR1C3 and clinical pathological characteristics in the gastric cancer by adopting the ?2. The result showed that the low expression of AKR1C3 is closely related to low degree of tumor differentiation(P=0.036), higher TNM-staging(P=0.046) and deep invasive degree(P=0.023), and showed no marked correlation with gender, age, tumor location, lymph node metastasis, vessel carcinoma embolus, nerve invasion, the size of tumor and other clinical and pathological characteristics(P > 0.05). 4. To carry out the survival analysis showed that the high expression of the AKR1C3 is marked related to better clinical prognosis of the patients with gastric cancer by adopting the Kaplan – Meir. The higher the expression of AKR1C3 led to the better prognosis(P=0.038). The result analysis of the single factor and multiple factors prognosis showed that the high expression of the AKR1C3 is the protective factor for the patients with gastric cancer who survive for a long term(P=0.041, HR=0.58; P=0.042, HR=0.57).Conclusion: 1. AKR1C3 is low expression in gastric cancer tissue. 2. The expression of AKR1C3 is closely related to the clinical pathological features, which playing an important role during the occurrence and development of gastric cancer. 3. High expression of AKR1C3 is closely related to the good prognosis of gastric cancer patients.
