Vitamin D Level In Children With Bronchial Asthma And Its Clinical Value

Aim: In order to improve vitamin D status and provide the theoretical basis for clinical practice, serum levels of Vitamin D were detected and several other clinical informations were collected in this study, the association between vitamin D and asthma was determined and the influence factors in vitamin D levels were analysed.Methods: According to diagnosis standards made by national pediatric asthma prevention and cooperation, 115 asthmatic children were included as patients' group during 2014.06 to 2015.10 in our department.During the same period, 63 healthy children were included as the control group in children's health examination in our hospital. 2m L blood was collected in both groups and vitamin D levels were determined using liquid chromatography-tandem mass spectrometry(LC-MS/MS) method, general information(gender, age, height and weight), diet habit, vitamin D supplement, outdoor time, parients' education, family financial situation and guardian information were collected, moreover, airway obstruction degree and asthma severity were evaluated. Besides, diagnosis standards made by National rachitis prevention research cooperation and The Chinese medical association branch of pediatrics(The child care group), vitamin D levels in both groups were evaluated. The SPPS13.0 was used to calculate the data, and values with P<0.05 were considered statistically significant.Results: The vitamin D3 levels were 18.74 and 26.96 ng/m L in asthmatic group and in control group, respectively, the total vitamin D levels were 19.31ng/m L and 28.08ng/m L in asthmatic group and in control group, respectively, compared to control group, asthmatic group has higher vitamin D3 and total vitamin D levels(P=0.000). 66.1% children in asthmatic group were vitamin D deficiency or insufficiency, which was much higher than control group(10.0%), differences between the two groups were significant(P=0.000). In asthmatic group, vitamin D levels had a significant positive correlation with airway obstruction degree and asthma severity, lower vitamin D and D3 levels indicated more serious airway obstruction degree and asthma severity, differences between each groups were observed(P=0.000). Firstly, single factor analysis was used to analyse the influence factors of vitamin D levels, the results indicted that vitamin D deficiency or insufficiency was related to diet habit, vitamin D supplement, outdoor time, family financial situation and groups. Then, non conditional Logistic multivariate regression analysis was used to analyse the risk factors above, the results indicated that diet habit(OR=7.270,95%CI:2.340~22.590,P=0.000), gorups(OR=2.937,95%CI:4.822 ~ 73.730, P=0.000) were risk factors of vitamin D levels, vitamin-D supplement(OR=0.039,95%CI:0.012~0.135,P=0.000), outdoor time(OR=0.108,95%CI:0.028~0.419,P=0.000) and financial situation(OR=0.246,95%CI:0.077~0.783,P=0.000) are protective factors. Moreover, multiple stepwise regression analysis were furtherly used to find risk factors of vitamin D levels, serum vitamin D levels of two groups and some factors were considered as the independent variable and dependent variable, respectively, the results suggested groups(B=-4.930,P=0.000)?outdoor time(B=3.495,P=0.000)?vitamin D supplement(B=3.335,P=0.000)?guardian(B=-3.245,P=0.000)?diet habit(B=-2.407,P=0.000) were included in the regression equation. in asthmatic group, serum vitamin D levels and some factors were considered as independent variable and dependent variable, respectively, the results suggested outdoor time(B=2.582,P=0.000)?vitamin D supplement(B=3.768,P=0.000)?parents' education(B=1.242,P=0.000)?guardian(B=-1.890,P=0.000)?diet habit(B=-3.097,P=0.000)?lung function(B=0.155,P=0.000) were included in the regression equation.Conclusion: Serum vitamin D of asthma children is lower than normal children, and about 66.1% asthma children were vitamin D deficiency or insufficiency; the vitamin D level is related to the severity of asthma and airway obstruction degree; the vitamin D level in asthma children is influenced by outdoor time, vitamin D supplement, parents' education, guardian, diet habit and lung function. Our results indicate vitamin D level should be valued in asthma children, and outdoor activit, vitamin D supplement, health education and asthma treatment should be encouraged in asthma children with low vitamin D level.
