Objective:Determination of serum iron and iron protein concentration in normal pregnant women during early and mid pregnancy.To investigate the predictive ability of iron and ferritin levels in the early stage of gestational diabetes mellitus,Search new ideas for early detection and prevention of gestational diabetes.Methods:Search from December 2013 to October 2015,Routine prenatal examination,physical health care,normal fasting blood glucose in the early stage of normal pregnancy and 300 cases of spontaneous pregnancy without any medication history in the Department of Obstetrics and Gynecology of Shanxi Academy Of Medical hospital, Determination of serum iron and ferritin concentration at 12 weeks gestation,Fasting blood glucose determination in 12 weeks of pregnancy,24~26 weeks line 75 g oral glucose tolerance test,According to the results of oral glucose tolerance test, pregnant women were divided into 50 cases of gestational diabetes group and 300 cases of normal control group.SPSS17.0 statistical software was used to analyze the results of the two groups of pregnant women,the results were based on the 0.05 as the test level.Results:1.The serum ferritin(59.3+8.4)?g/L in GDM group was higher than that in normal pregnancy group(37.4 + 6.7) ?g/L, and the difference was statistically significant(P<0.05).2.The serum iron element(36+7) mmol/L in GDM group was higher than that in normal pregnancy group(28.4+8.5) mmol/L, and the difference was statistically significant(P<0.05).3.GDM group of 12 weeks pregnant serum ferritin ?81.8?g/L less than the development of 81.8?g/L for gestational diabetes,the difference was statistically significant(P<0.05).4.GDM group of 12 weeks pregnant serum iron element ? 32.4 mmol/L less than the development of 32.4 mmol/L for gestational diabetes,the difference was statistically significant.5.The area under the ROC curve of 12 weeks pregnant women with serum ferritin in the prediction of gestational diabetes mellitus was 0.955,the best critical Value(cut off value) was 71.8 ?g/L,at this time sensitivity(SE) for 0. 855,specificity(SP) for 0.792,positive predictive value(PPV) for 0.935,negative predictive value(NPV) is 0.596,positive likelihood ratio(+ LR) for 4.792 and negative likelihood ratio(LR) is 0.291.6.The area under the ROC curve of weeks pregnant 12 weeks of serum iron in predicting gestational diabetes mellitus was 0.898. Value(cut off value) for 48.9mmol/L.At this time at this time sensitivity(SE) for 0. 877,specificity(SP) for 0.763, positive predictive values(PPV) was 0.942, negative predictive value(NPV) is 0.576, positive likelihood ratio(+ LR) for 4 638 and negative likelihood ratio(LR) is 0.258.Conclusion:There is a certain correlation between the high concentration of serum iron and ferritin and the occurrence of gestational diabetes,Early detection of serum iron and ferritin levels can improve the detection rate of gestational diabetes mellitus,In the early the increase of SF,and ron in pregnant women should be alert to the occurrence of GDM.
