Objectives To investigate the distribution and major influencing factors of annual systolic blood pressure variability from a large population cohort.Methods In our study, data from Kailuan Group employees who attended all 4 physical examinations(taken in June 2006 to October 2007, June 2008 to October 2009, June 2010 to October 2011, June 2012 to October 2013, respectively) were analyzed(32 959 males and 10 401 females, mean age:(48.2±11.5) years old).Blood pressure(BP) was measured on the left arm using a mercury sphygmo-manometer following the standard recommended procedures.Systolic blood pressure variability was defined as the standard deviation(SSD)and the coefficient of variation( SCV) of systolic blood pressure of 4 physical examinations. Multivariate linear regression analysis was used to determine the related influencing factors of SSD and SCV.Results 1 The mean of SSD and SCV for this cohort were10.91 mmHg(1 mmH g=0.133 k Pa) and 8.34%, respectively,the 95% Confidence Interval were 2.50~24.83 mm Hg and 2.06~17.49%. 2 The observation population was divided into five gro ups by age per 10 years interval, Both SSD and SCV increased in male and femal e with increasing age(P<0.001). 3 Multiple linear regression analysis showed that systolic blood pressure( ?=0.218,P<0.001), age( ?=0.157,P<0.001), female(?=0.044,P<0.001), fasting blood glucose(?=0.038,P<0.001), the use of anti-hypertensive drugs(?=0.024, P<0.001), alcohol drinking(?=-0.021,P=0.001), smoking(?=0.020,P<0.001),serum uric acid(?=0.019,P<0.001), total cholesterol(?=0.019,P<0.001), high-sensitivit y c-reactive protein(?=0.016,P=0.001), physical exercise(?=-0.015,P=0.001) and body mass index(?=-0.011,P=0.027) were related to SSD. Age(?=0.136,P<0.001), female(?=0.058,P<0.001), systolic blood pressure( ?=0.042,P<0.001), fasting blood glucose(?=0.032,P<0.001), body mass index(?=-0.028,P<0.001), alcohol drinking(?=-0.027,P=0.001), smoking(?=0.020,P=0.001), the use of anti-hypertensive drugs(?=0.016, P=0.001), total cholesterol(?=0.015,P=0.005), physical exercise(?=-0.014,P=0.005), high-sen sitivity c-reactive protein(?=0.013,P=0.006) and serum uric acid(?=0.011,P=0.032)were related to SCV.Conclusions SSD and SCV increase with increasing age. Age, the female, systolic blood pressure, fasting blood glucose, high-sensitivity c-reactive protein, total cholesterol, serum uric acid,smoking and use of anti-hypertensive drugs are positively correlated with SBPV,whereas drinking, physical exercise, body mass index are negatively correlated with SBPV.
