Objectives To explore the high risk factors of bone mass change and the characteristics of bone transformation index in patients with hemophilia. Methods 23 patients with hemophilia were enrolled from the Hospital of North China University of science and technology from March to August, in 2015, 28 healthy men as control group who are all male and similar age with hemophilia. All accepted the conversion of bone markers detection, including two makers of bone metabolism:serum I collagen cross-linking N terminal peptide(NTX I) and bone alkaline phosphatase(BALP), four cytokines regulating bone metabolism: Osteoprotegerin(OPG), Basic Fibroblast Growth Factor(b FGF), Insulin-like growth factor(IGF) and Transforming growth factor-?1(TGF-?1). 21 hemophilia patients completed the form of questionnaire and the score scale of activity ability released by the U.S. Center for disease prevention and control in 2002,received bone mineral density measurement. According to the World Health Organization(WHO) definition, the clinical significance of bone mineral density(BMD) was assessed by measuring the level of Z. Statistical methods of t test and bivariate correlation test were used to test the correlation results. Results There are 10 cases in the level of Z>-2,11 cases Z?-2; The level of bo dy mass index(BMI) in the Z?-2 group is lower than the Z>-2group, but the scor e of activity ability of two groups have no difference. The level of BALP(697.92±151.12pg/ml) and NTXI(41.53±9.33ng/L) and b FGF(1431.36±346.86ng/L) of c ontrol group are significantly higher than that of hemophilia group(P<0.05), The r est of the three measures of bone transformation have no statistical difference bet ween the two groups(P>0.05). The level of BALP of Z>-2 group(677.23±98.96pg/ml) is significantly higher than that of Z?-2 group(547.07±130.44pg/ml)P=0.019, and the level of BALP(r=0.489,P=0.024) have positively correlation with BMD(P<0.05); The levels of NTXI have no statistical difference between Z?-2 group and Z>-2 group(P>0.05). In patients with hemophilia, the levels of four cytokines ha ve no significant difference between Z?-2 group and Z>-2 group(P>0.05). The le vels of b FGF(r=0.570,P=0.007) and OPG(r=0.505,P=0.02) are positively correlated with NTX?which reacting bone destruction(P<0.05), while the four cytokines had no correlation with BALP(P>0.05);Conclusions 1 The activity of osteoblast and osteoclast of hemophilia patients is decreased, bone mass losing for hemophilia patients may be mainly related to the decreasing of osteoblast activity; 2 The b FGF and OPG might promote bone metabolism of patients with hemophila;...
