Objective:To explore the influence of early intensive BP-lowering treatment on hematoma volume and perihematoma edema in patients with Hypertensive intracerebral hemorrhage.Methods:Retrospective analysis the data from 2013 to June 2015 of 180 supratentorial hypertensive intracerebral hemorrhage patients who had hypertensive intracerebral hemorrhage treatment within 6 h of onset. According to the different methods of blood pressure management,all the patients dinided into intensively antihypertensive group(with a target systolic level of <140 mm Hg within 3 hour) and control group(slowly antihypertensive therapy when systolic BP>180mm Hg,or mean arterial pressure > 130 mm Hg. target systolic BP 160 ~ 180 mm Hg and mean arterial pressure was100~130mm Hg). Two groups according to age, GCS, systolic blood pressure, after the onset of the intervention time, hematoma volume is divided into five aspects,each aspect is divided into two subgroups, and make a comparison, Compar the incidence rate of hematoma expansion in three days and calculated the relative risk reduction(RRR).Compare the edema volume at 3d,7d,14 d of the two groups.Results: hematoma enlargement happened in 9 cases in the intensively antihypertensive group(10%) and in 24 cases in the control group(26.7%). the difference between the two groups was statistically significant(P<0.01). RRR=62.5%(95%CI 23.9%~81.5%). at 3d,7d,14 d in the two groups, the brain edema difference was no statistical(P>0.05).Conclusion: Early intensive BP-lowering treatment is helpful to control hematoma enlargement and do not exacerbate edema growth.
