Objective: To observe the changes in blood pressure after meals in hospitalized essential hypertensive(EH) patients to explore the relationship between postprandial hypotension(PPH) and the time of taking anti-hypertension medicine in those patients.Methods: Fifty-four hospitalized patients aged 58.9±11.1 years with EH were recruited in this study, including 30 men and 24 women. The duration of disease was 6.9±5.8 years. The patients accepted one or more anti-hypertensive medicine treatment and took medicine before breakfast. The blood pressure(BP) and heart rate(HR) were measured 5 minutes before meals and every 15 minutes within 2 hours after meals. Patients were divided into two groups according to mean values of systolic blood pressure(SBP) after breakfast: postprandial hypotension group(PPH, n=20) and non-postprandial hypotension(NPPH, n=34). Blood biochemical indicator and echocardiographic parameters were collected. To regulating the medication time according to BP and HR. The patients taking the medicine with PPH before breakfast adjusted the medication time to 2h after breakfast, then continued to monitor BP and HR to compare with taking the medicine with EH before breakfast.Results: The detection rate of post-breakfast and post-lunch PPH in all patients was 37.04% and 28.26%, respectively. The detection rate of PPH and the decline of SBP were elevated accompanied with theincrease of ageand basic SBP before breakfast(P<0.05). The detection rate of post-breakfast and post-lunch PPH declined to 30.00% and 25.00%, respectively(P<0.05) after regulating the medication time. There was a significant positive relationship between the decrease in SBP after breakfast and age(r=0.515,P<0.05), basic SBP before breakfast(r=0.609,P<0.01). Logistic regressionanalysis indicated that the age and the basic BP before breakfast were independent determinant of PPH in hospitalized patients with EH.Conclusion: The detection rate of PPH in hospitalized EH patients is higher. The detection rate of PPH may be lowed for anti-hypertension medication patients 2 hoursafter breakfast.The age and basic SBP before breakfast are the independent risk factors of PPH in hospitalized patients with EH. |
