Objective To Summary the treatment of moderately severe acute pancreatitis(MSAP)and severe acute pancreatitis(SAP),and establish the minimally invasive treatment system and process of MSAP and SAP.Methods Retrospectively analyse of 417 SAP cases, admitted in Fujian medical university union hospital from March 2008 to May 2015, Who were divided into early group(n=131)and recent group( n =286).The recent group received integrated new minimally invasive treatment system, while the early group received conventional treatment.To statistic the baseline data of the patients, such as age, gender, incentives, APACHE–II score, and MCTSI score.Compare differences of the hospitalization days, operation rate, mortality and complications probability of the two groups.Results1. The early group consist of 71 males and 60 females,aged from 11 to 87, Average(43.67± 16.14), APACHE II score 11.67±4.12, MCTSI score 6.26±1.32. While the recent group consist of 172 males and 114 females, aged from 13 to 93, Average(45.7 ± 16.84),APACHE II score 12.32±3.92, MCTSI score 6.10±1.02; There was no statistically significant difference about the age, sex, disease severity between the two groups.2. The main causes of the early group were biliary diseases(39cases,29.8%), alcohol(27cases,20.6%), outeating(24 cases,18.3%),and hyperlipidemia(23 cases,17.6%).Of a11 the patients of the recent group,94 were induced by biliary diseases,65 were induced by outeating, while 57 were induced by alcohol,and 38 were induced by hyperlipidemia. The major causes of the two groups were similar, and there was no statistically significant difference about them.3.The recent group had a significant low rate of operation(23.42% VS 43.51%,P <O.05)and mortality(4.89% VS.8.40%,P < 0.05). In addition,the recent group costed shorter hospital duration[(26.83±24.52)d VS(31.67±26.34)d,P<0.05] and low rate of complications(41.95% VS 79.39%).Conclusion Establish a standardized system of minimally invasive treatment for MSAP and SAP patients can help to shorten hospital stay, decrease the operation rate?the incidences of complications and mortality.
