Objectives:Yanan city in China is the high incidence of gastric cancer.The purpose of this study is to investigate the epidemiological and clinical characteristics of Yanan city and change trend in recent 10 years the incidence of gastric cancer,improve the understanding of the local population of gastric cancer, gastric cancer prevention and provide reference for the development of scientific strategy.Methods:1.January 2006-2015 year in December nearly 10 years hospitalized in Affiliated Hospital of Yan'an University, combined with endoscopic diagnosis and pathological diagnosis of gastric cancer cases, which can be divided into before and after the January2006-2010 year in December and January 2011-2015 year in December five years in two time periods, respectively, for A and B two groups.2.Comparative analysis of two groups of gastric cancer incidence rate, age, gender,region, occupation distribution, endoscopic diagnosis, pathological features and clinical epidemiological features and change trend whether Hp infection.Results:1.A was detected in 985 cases of gastric cancer, the detection rate was 2.73%, B was detected in 1301 cases of gastric cancer, the detection rate was 2.31%, B group compared with A group, the detection rate decreased(P < 0.05), the difference was statistically significant.2.Analysis of 2 general information: two groups in the sex ratio of the difference was not statistically significant(P > 0.05). There are significant differences between the two age groups(P < 0.05). Occupation distribution of two groups of patients with gastric cancer were accounted for a higher proportion of farmers. Two groups of patients with gastric cancer in different regions of the difference was not statistically significant(P>0.05).3.Analysis from the onset of a statistically significant difference in the incidence of different parts of the composition of the two groups(P < 0.05). B group of gastric cardia carcinoma and gastric cancer detection rate than A group increased(P < 0.05), subtotal or total detected rate of gastric cancer has decreased(P < 0.05), gastric cancer and gastric stump cancer in five years before and after the detection rate had no significant difference(P > 0.05). Two groups of patients with gastric cancer in male and female gastrectomy or total gastric cancer in the proportion of the difference(P < 0.05); and gender in gastric cardia cancer, gastric cancer, gastric cancer and gastric stump cancer in constituent ratio had no significant difference(P > 0.05). Two groups of patients with gastric cancer in different parts of the constituent ratio of age had no significant difference(P > 0.05).4.From endoscopic type analysis: there was no significant difference of two group of gastric cancer patients with different types of microscope(P > 0.05). The detection rate of the two groups of patients with early gastric cancer and advanced gastric cancer,there were no significant differences(P > 0.05). Two groups of patients with gastric cancer in ulcerative type in gender composition ratio difference(P < 0.05); and in the early gastric cancer, polypoid carcinoma, infiltrative carcinoma, diffuse infiltrative ulcer sex type cancer ratio has no obvious difference, P values were greater than 0.05. Two groups of patients with gastric cancer in ulcerative carcinoma of young, middle-aged and older age of three groups of difference ratio(P < 0.05), in other endoscopic type in three groups had no significant difference in age structure, P values were greater than 0.05.5.From the analysis of pathologic type: two groups of patients with gastric cancer in different pathological types of no significant difference(P > 0.05). B group of signet ring cell carcinoma detection rate compared with the A group increased, the difference was statistically significant(P < 0.05); carcinoma, mucinous cell carcinoma and other types of gastric cancer before and after five years the detection rate, the difference was not statistically significant, P values were greater than 0.05. Two groups of patients with gastric cancer in different pathological types in gender composition than were statistically significant, P values were less than 0.05. Two groups of patients with gastric cancer in signet ring cell carcinoma in youth and middle aged group, three age differences in the constituent ratio(P < 0.05), in the rest of the three kinds of pathological type in three groups had no significant difference in age structure, P values were greater than 0.05.6.From Hp infection analysis, two groups of patients with gastric cancer Hp positive rate decreased, the difference was statistically significant(P < 0.05). Two groups of patients with gastric cancer regardless of gender Hp positive and negative were different(P < 0.05), the difference was statistically significant. Two groups of patients with gastric cancer in Hp positive infection age structure has no obvious difference, P value is greater than 0.05. A, B two groups of patients with gastric cancer in Hp negative infection in different age structure(P < 0.05), the difference was statistically significant.Conclusion:1.detection rate: the detected rate of gastric cancer has decreased;2.general information: incidence of male is higher than female, two groups of men and women of different ages are still higher than; in elderly group high detection rate, the average age of farmers increased; the highest incidence rate.3.location: two groups of predilection sites were subtotal or total gastrectomy,followed by gastric antrum. Gastric cardia carcinoma and gastric cancer detection rate is on the rise, gastrectomy or total detected rate of gastric cancer has decreased, the incidence of gastric cancer to the proximal part of all parts are to move; elderly men,cardia cancer sex ratio increased significantly, the number of men with gastric cardia carcinoma increased; with the increase of age, the number of detected cases in different parts of gastric cancer were increased.4.endoscopic type: two groups were infiltrative ulcer mostly, early gastric cancer detection rate is low; the type of gastric cancer are the most common in elderly men,ulcer type cancer sex ratio increased significantly, the number of male ulcerative carcinoma has an increasing trend. With the increase of age, the number of detected cases of gastric cancer were increased.5.pathological types: two groups were often adenocarcinoma, elderly male, signet ring cell carcinoma detection rate is on the rise. With the increase of age, the pathological type of gastric cancer patients with positive number increased.6.Hp infection: two group Hp positive rate decreased, both positive and negative Hp infection, males were more than females in the middle aged and elderly males.
