Clinical Effect Of Da Cheng Qi Tang On Inpatients Of Severe Acute Pancreatitis With Intra-abdominal Hypertension

Objective:Observe the effects of Da Cheng Qi Tang to the inpatients of severe acute pancreatitis(SAP)with intra-abdominal hypertension.Method:Subjects in the study are all the inpatients from icu in the hospital of shan dong qianfo(2015.2.14—2016.2.27).The presupposed sample size is 40,and accoding to the <Guidelines for the diagnosis of acute pancreatitis(2014)>,we put them into treatment group(n=20) and contrast group(n=20) in the retrospective analysis.The contrast group,we give conmbined treatment(general treatment<relieve spasm,protease inhibitor,pancreatin inhibitor>,fluid resuscitation,maintenance of organ function,blood purification,nutritional support).The treatment group,on the foundation of the combined treatment,we also give the patients who meet the criterion the Da Cheng Qi Tang through Nj-en after 72 hours since the paroxysm of SAP(rheum officinale 12 g, mangnolia officinalis 24 g,fructus aurantii immaturus 12 g,glauber salt6 g,take decocted with water 200 ml,divide it into two parts and give it to the inpatients morning and evening),the treatment cycle is 7 days.Observe the change of the related detection indexes(serum amylase.blood lipase.leucocyte count.renal function)and the MCTSI and the change of IAP of inpatients with IAH,figure out the average stay.Result: There are not significant differences in general signs,age,gender,pathogeny,related detection,severity and complication of patients in the two group before treantment, so they are comparable. Until the end of the observation,the improvement of related detection indexes,patients in the treatment group,requires less time,and the average stay of patients in the treatment group are obviously short when compared with the contrast group.The decline of the IAP,inpatients with IAH in two groups,of the inpatients in the treatment group is faster than that in the contrast group,but no significant differences of MCTSI were found in two groups.Conclusion:On the foundation of the western medicine combined treantments,we should use the Da Cheng Qi Tang in the treatment of patients of SAP with IAH.Since it can reduce the IAP,and shorten the recovery time of related detection indexes and the average stay,and benefit patients a lot.
