| ObjectiveThrough comparative observe the efficacies of pedicle screw fixation through the pedicle of fractured vertebra plus short-segment pedicle instrumentation and traditional short-segment pedicle instrumentation in the treatment of thoracolumbar fractures,to discuss the mechanism,advantages and indications of pedicle screw fixation through the pedicle of fractured vertebra plus short-segment pedicle instrumentation,to To provide a way of thinking for clinical treatment.MethodsRetrospectively analysis the 60 cases of single segmental thoracolumbar fracture between September 2013 and January 2015 treated in Orthopaedic Hospital Of Zhengzhou by posterior operation,divided them into two groups.Group A consisted of 28 patients treated by pedicle screw fixation through the pedicle of fractured vertebra plus short-segment pedicle instrumentation.Group B consisted of 32 patients treated by traditional short-segment pedicle instrumentation.Record separately the length of operation time and operation bleeding volume,measure and compare separately the data of anterior body height compression,the sagittal Cobb ’ s angle,Frankel classification and VAS score preoperatively,postoperatively,and at the last follow-up.Record the situation of loosening and fracture of the internal fixation.Analysis these datas statistically.ResultsAll cases were followed up.There was no significant difference(P>0.05)between the two groups in age,gender,cause of injury,injured segment,type of fracture,Frankel classification,length of stay in hospital and follow-up time.The follow-up period was 16 months on average,from 12 to 24 months.There were no internal fixation loosening and fracture occured.There was no significant difference(P>0.05)between the two groups in the length of operation time and operation bleeding volume,as well as the data of anterior body height compression,the sagittal Cobb’s angle and VAS scores preoperatively.As for the former two factors,There was no significant difference(P>0.05)between the two groups postoperatively.while the difference was significant(P < 0.05)at the last follow-up.All the three factors were improved postoperatively(P<0.05)in both groups compared with those preoperatively.There was a loss in the data of anterior body height compression and correction of the sagittal Cobb’s angle at the last follow-up when compared with those just after the operation in both groups,and the difference between the two groups was significant(P<0.05).Conclusionspedicle screw fixation through the pedicle of fractured vertebra plus short-segment pedicle instrumentation can effectively correct the deformity,restore the anterior body height,and have an advantage compared with traditional short-segment pedicle instrumentation in avoidance of the loss in data of anterior body height compression and correction of the sagittal Cobb’s angle,and is is worth promoting. |
