This study, from two aspects of clinical and experimental observation of heat discharge polluted the clinical curative effect of treating gouty nephropathy, and its mechanism are discussed in this paper, for provides objective theoretical basis for its clinical application. Clinical researchPurpose:Observation on the heat leakage of turbidity party gouty nephropathy patients with clinical symptoms of TCM and laboratory indexes to improve the situation.Methods:Elected to 50 cases in accordance with gouty nephropathy patients with diagnostic criteria of western medicine. 30 cases were randomly divided into heat discharge turbidity party group and the control group 20 cases, control group treated with allopurinol + bicarbonate of soda, clear heat discharge turbidity party group on the basis of treatment with clear heat discharge turbidity party, a course of 4 weeks.Results:1. Clearing heat and relieving turbidity party group total effective rate was 90%, control group total effective rate was 65%. The effect of Clearing heat and relieving turbidityparty group was obviously better than the control group(p < 0.05) than the control group(p < 0.05). 2. Clearing heat and relieving turbidity syndrome score comparison before and after the treatment group, was obviously improved after treatment(P < 0.01); The control group syndrome score slightly improved but no significant difference(P > 0.05). Clearing heat and relieving turbidity party group is compared between two groups after treatment syndrome score was significantly better than the control group(P < 0.01).3.Clearing heat and relieving turbidity party set quantitative urine protein, blood uric acid(UA), creatinine(Scr), urea(BUN) before treatment were significantly decreased(p < 0.01). The control of the Scr, BUN, before the treatment has no obvious change(p > 0.05).4. The two groups are not severe side effects in the treatment process.Experimental study Purpose:Research heat discharge turbidity party UA of gouty nephropathy rats, Scr, BUN, and kidney tissues alkaline fibroblast growth factor, basic fibroblast growth factor, b FGF)- 2 and oxidase(Cyclooxygenase, cox-2), and observe its effect on rat renal pathology, so as to explore its action mechanism for the treatment of the disease.Methods:To 40 male rats were randomly divided into 4 groups, the normal group, model group, Chinese medicine group, the allopurinol group, in addition to the normal group, more than four groups are using yeast extract and oxygen oxazine acid potassium(OXO) share building, building after successful living heart blood tests the UA, Scr, BUN. Intervention process: normal group, model group, with 3 ml saline lavage, allopurinol allopurinol group to 0.01 g/kg.(d) 3 ml lavage, Chinese traditional medicine group to clear heat discharge turbidity 3 ml lavage. 2 weeks after the intervention, abdominal aorta take blood test the UA, Scr, BUN, kidney of rats, on the kidney tissue HE staining to observe the pathological change, application of immunohistochemical technique to detect the rat kidney b FGF and cox-2 expression.Results:1.Compared with normal group, building groups of UA increased significantly(p < 0.01). 14 days later, the model group compared with normal group of UA still increased obviously(p < 0.01). Compared with the model group after 14 days, after the intervention of two groups of UA were significantly lower(p < 0.01).2.In addition to the normal group, after the value of the building groups of Scr and BUN were significantly increased(p < 0.01). 14 days after drug intervention, Scr and BUN values are still of model group was obviously higher than in normal group(p < 0.01) in traditional Chinese medicine and western medicine group, Scr and BUN of the 14 days of model group decreased, but the Chinese medicine group of reduce BUN is more obvious in the western medicine group(p < 0.05). 3.The pathological: building groups appear renal tubular expansion, renal tubular epithelial cells around urate crystal deposition, and can be observed in renal tubular interstitial inflammatory cell infiltration. Heat discharge turbidity party group and the renal tubular expansion of allopurinol group compared with model group. And two groups of urate crystal deposition and inflammatory cells infiltration quantity are compared with model group.4.Compared with normal group, model group b FGF, cox-2 expression were significantly higher(p < 0.01), two treatment group b FGF and cox-2 expression were significantly lower in the model group(p < 0.01), and Chinese medicine group decreased obviously.Conclusion:1.Clear heat discharge turbidity party group can obviously improve clinical symptoms in patients with gouty nephropathy, reduce the 24 h urine protein quantitatively, the UA, Scr, BUN, and no obvious side effects.2.The heat leakage of turbidity can reduce the UA, Scr and BUN levels in rats, improve the renal pathological damage, its mechanism may be related to reduce the expression of b FGF and cox-2 in the kidney.
