Objective: To explore the relationship between the polymorphism of beta adrenergic receptor gene and susceptibility to EV71 infection in Hand,Foot and Mouth Disease.To analyze the clinical significance of blood catecholamine levels in children with enterovirus 71 infection in HFMD.Methods:This study research 57 HFMD patients in hospital, according to the clinical manifestations.they are divided into mild HFMD group inciude 22 examples,severe HFMD group inciude 27 examples and critically HFMD group inciude 8 examples,specimen collection was asked consent of parents 20 examples, All cases admitted to hospital were taked 3ml from peripheral blood of children and healthy children, Centrifuge is divided into blood cells and plasma in-70℃ refrigerator preservation.(1) blood cells:Using PCR in vitro amplification technology, the PCR products were detected by Sanger sequencing method to detect the 5 single nucleotide polymorphisms of beta adrenergic receptor, include of ADRB1 Ser49 Gly,Arg389Gly,ADRB2 Argl6 Gly,Gln27Glu,ADRB3 Trp64 Arg gene.(2) plasma: enzyme linked immunosorbent assay(ELISA) was used to determination of hand, foot and mouth disease mild, severe and critical disease groups catecholamine levels, at the same time by the same method determination of healthy children group(control group).Result:(1)The 57 cases of EV71 infection group according to the severity of the disease is divided into mild group and severe group and critical group,plasma epinephrine,norepinephrine and dopamine concentrations were higher than the normal control group,there were significant differences between the groups(P<0.05);mild group and control group of plasma epinephrine and norepinephrine concentrations compared with difference(z=-2.972,-4.105,P=0.003,0.000), no significant differences between the concentration of dopamine(z=-0.201, P=0.840); mild group and severe group plasma epinephrine concentration differences(z=-1.995,P=0.046),norepinephrine and dopamine concentrations showed no significant difference(z=-0.493,-0.915, P= 0.622,0.360);severe group and critical group of plasma epinephrine, norepinephrine and dopamine concentrations were the difference(z values were-3.377,-2.392,-3.110, P values were 0.001, 0.003, 0.017).(2)The genotype frequencies of CC,CG and GG of ADRB1 Arg389 Gly was 31.6%,31.6%,19.3% in case and 60.0%,35.0%,5.0% in control group. There was not statistically significant between the EV71 group and the normal group(P>0.05).The frequencies of C and G alleles were 56.1%, 43.9% in case and 77.5%, 22.5% in control group,the difference was statistically significant(P<0.05). There other gene sites the frequency of genetype and allele in the two groups was both not statistically significant.(3)There were no significant differences in the distribution of frequency of beta adrenergic receptor gene genotype and allele frequency between the mild HFMD group,severe HFMD group and critically HFMD group(P> 0.05).Conclusion:(1) Plasma catecholamine concentration levels with the severity of the disease increased, catecholamine concentrations of epinephrine can be used as an important auxiliary examination index and predictive factors of hand foot and mouth disease judgment.(2)The ADRB1 Arg389 Gly polymorphism might be associated with in EV71 infection, Carrying the G allele may increase the risk of EV71 infection, But the severity of EV71 infection was not related to β-AR gene polymorphism.(3)The Gly389 allele of EV71 infection in plasma norepinephrine concentration level was significantly higher than that of Arg389. |
