Objective : To investigate the status, genotype distribution and change characteristics of hepatitis C virus(HCV) infection among general patients(GP) and injecting drug users(IDUs) in Chenzhou.Methods: 1. Serum samples from GP and IDUs, Elisa was applied to test anti-HCV, HBs Ag and anti-HDV among different population.2. Nest reverse transcription polymerase chain reaction(RT-PCR)was detected by HDV RNA and HCV RNA( 5'UTR-C and NS5 B gene),and the HCV RNA positive product was sequenced.3. Serum HCV RNA was detected by real-time PCR among different population.4. The HCV 5'UTR-C and NS5 B gene sequences were aligned, and phylogenetic trees were reconstructed with Mega 5.2 software.Results: 1. HCV RNA positive rate were 64.6% in general patients and 81.1% in injectiong drug users infected with HCV Ig G. HCV mono-infection rate were 52% and 71%, respectively; co-infected with HCV and hepatitis B virus(HBV) and hepatitis D virus(HDV) rate were0.8% and2.2%,respectively; co-infected with HCV and HBV rate were5.6%, respectively.2. The genotype 1b, 2a, 3 and 6a were 27(32.9%), 8(9.8%),19(23.2%)and 22(26.8%), respectively, in general population;but the genotype1 b, 2a, 3 and 6a were 8(3.7%), 8(3.7%), 70(32%)and 129( 59.2%), respectively, in injecting drug users. 1b strain dispersed among strains from Guangzhou and Gunagxi, 2a strains were closely clustered with strains from Hubei, but 6a strains showed no relationship to any of the original strains through phylogenetic trees.3. Among different population, the HCV RNA level had statistically significant differences in different genotype. In GP, the patients infected with genotype 6a had higher HCV RNA than those with genotype 3 and genotype 1b. In IDUs, the patients infected with genotype 6a had higher HCV RNA than those with genotype 3 and genotype 2a, and the patients infected with 2a had lower HCV RNA than those with genotype 2a. The HCV RNA level had statistically significant differences in patients single infected with HCV and those with HCV and HBV co-infection(all P<0.05).4. Among different population, the ALT and AST had no statistically differences in different genotype, and the ALT and AST level had no statistically differences in patients single infected with HCV and those with HCV and HBV co-infection(all P>0.05).5. Through the correlation analysis, HCV RNA had no correlation with ALT or AST.Conclusion: 1. The main HCV genotype is 1b in general population;however,the main HCV genotype is 6a in injecting drug users.2. The general population patients had higher level of HCV RNA than those of the injecting drug users, and HBV and HCV co-infection had different HCV replication level among different population.
