ObjectivesTo describe the epidemiological characteristics of measles from 2005 to2014 in Zhumadian city;explore its influencing factors, examine the cognitive situation of measles among the residents, and summarize progress towards measles control and elimination.MethodsThe datum of measles cases of the city from 2005 to 2014 was collected and epidemiological method was adopted to describe its temporal, endemic and population distribution. The incident rate and 18 indicators of 194 townships or street offices of the city in 2010 consist of natural and social factors were collected, and the association between measles and related factors were conducted to evaluate by one-way ANOVA and two-level binary logistic regression. Questionnaire survey was conducted among 1230 residents recruited by multi-stage sampling to understand the status of measles knowledge.The correlation between measles incidence and cognition rate of measles among the residents was conducted to evaluate by Linear Correlation Method.Results1. From 2005 to 2014, A total of 3645 measles cases were reported,including 11 deaths, the average annual incidence, fatality rate and mortality rate was 4.22/lakh,0.30/100 and 0.01/lakh respectively,all lower than Henan province and the whole country during the same period. Sporadic and outbreaks exist at the same time. Measles shows seasonal distribution characteristic, a higher proportion of cases occurred in winter and spring. the age specific incidence was highest among children aged<5 years,and the proportions of measles cases aged 0-1 year old increased recently.2. Multivariate analysis confirmed that there was positive correlation between the prevalence of measles with population density and proportion of floating people, negative with Per capita GDP, coverage of MCV and number of health and epidemic prevention staff.There is linear correlation between measles and cognition rate of measles among the residents.3. The overall cognition rate of residents on measles was 70.09%.The difference of residents? knowledge level among the different age, gender,education groups, occupation and income groups have statistics significance( P<0.05), but no statistics significance between different race.The main ways residents get to know the knowledge of measles prevention and control were relatives(36.24%), mass median(32.22%) and primary health institution(26.32%). Most people are actively to know more about measles and trust the current measles elimination strategies and measures taken by thegovernment.Conclusions1. Though both the local incidence and mortality rate of measles were involved into low level,it is still a certain distance from the goal of elimination disparities. There is an upward trend in the incidence of measles in recent years,and a new feature of the disease that there is an great increasing of proportions of measles cases aged 0-1 year old presents, record as unilateral "two-way shift" phenomenon simply.2. Measles is influenced by multiple social factors such as demographic factors, economic development level, human resources for health, immune prevention measures and effects of health education and health promotion and so on.3. The overall cognition rate of residents on measles was 70.09%.To improve cognitive level related measles of residents, It is necessary to strengthen the health education and health promotion of residents, especially the low-income families, male residents and migrant population.
