Stroke also known as Cerebralvacular accident, is group of nervous system diseases, characterized as blood vesssels supplied the brain blood disorders,including cerebral thrombosis, cerebral embolism, cerebral hemorrhage, subarachnoid hemorrhage, etc. Stroke has the characteristics of high incidence, recurrence rate, high morbidity and high mortality. Statistics suggest that the mortality rate of cerebrovascular disease in 2014 China urban area was 125.56 per one hundred thousand, the rate in rural residents was 150.17 per one hundred thousand. So the stroke has been a serious public health problem, the prevention and control is very urgency in China.Obesity, hypertension, dyslipidemia, hyperglycemia, excessive drinking, lack of physical activity are associated with stroke occurs, but the factors the pathogenesis of stroke is also associated with genetic factors. Abnormal lipid metabolism is the important causes for the atherosclerosis, leading to the occurrence of disease of heart head blood-vessel. AQP7and AQP9, which belong to the family of aquaporins, play important roles in the processes such as glycerol transportation and energy metabolism. In recent years, researchers begin to pay attention to the associations between AQPs genetic variations and disease susceptibility. The topic is based on people queue follow-up survey in hypertension community in Jiangsu Provence, to explore the associations between gene variants of AQP7 and AQP9 and the risk of stroke, using a nested case-control design. Part I The nested case-control study on the risk factors of stroke in patients in Jiangsu Provence hypertension community Objectives To know the incidence of stroke and the related influencing factors in hypertension community in Jiangsu Provence.The topic were followed-up annually in hypertension community from January 2010 to December 2014. This study will explore the risk factors of stroke and provide scientific evidence for further health intervention for community population. Methods Based on the hypertendion community cohort study, the people with first occurrence of stroke as the cases, people with non-occurrence of stroke with high blood pressure as the control group, case-control study was carried out by 1:3 matched with the age(±3years old),gender, region. Results 1.The total of 331 number of patients with stroke were collected, cumulative incidence of 2.68%, the patients were 993 in control group, total of 1324 people was gathered in case-control group. 2.Hypertension duration, high fasting blood glucose, high triglycerides, abnormal high-density lipoprotein cholesterol and systolic blood pressure are risk factors for stroke occurrence, the high fasting blood glucose is an important risk factor for stroke(OR=2.70,95%CI:1.774-4.108).Conclusions In a variety of risk factors of stroke, high fasting blood glucose and high triglycerides are important risk factors, we should strengthen the intervention in hypertensive population.Part ? Associations between AQP7, AQP9 gene polymorphisms and risk of strokeObjectives To illuminate the associations between genetic variants in AQP7 and AQP9 genes and stroke in Chinese, and explore the possible gene-gene and gene-environment interactions.Methods Baseline blood samples were taken from stroke cases and controls. Genomic DNA was extracted by DNA Blood Mini Kit (QIAGEN). Genotyping of 5 SNPs in AQP7 (rs2989924, rs3758269, rs2542743) and AQP9 (rs57139208, rs16939881) was performed by the polymerase chain reaction assay with TaqMan probes.Results 1.The results showed that SNP rs2989924 was associated with the risk of stroke in case-control study, the different frequency distribution was statistically significant. Compared to rs2989924 AA+AG genotype, GG genotype significantly increased the risk of stroke, with an adjusted OR of 1.741(95% CI:1.232-2.461); after adjusted confounders, rs3758269 CC+TT genotype can decreased the risk of stroke, with an adjusted OR of 0.669 (95% CI: 0.450-0.994), compared to rs3758269 CC genotype.2.Based on the stratified analysis, we found that SNP rs2989924 significantly increase the risk of stroke in male, over 60 years old and overweight subjects, with an adjusted OR of 2.23 (95% CI:1.32-3.77),2.20 (95% CI:1.41-3.44),2.35(95% CI:1.45-3.81) and 1.81(95% CI:1.22-2.70) in implicit model. Before adjusted related factors SNP rs3758269 has nothing to do with the stroke,but after adjusted related factors, SNP rs3758269 was associated with decreased risk of stroke in over 60 years old and overweight subjects, with an adjusted OR of 0.48 (95% CI:0.25-0.94) and 0.43(95% CI:0.24-0.77) in dominant model.3. rs2542743?rs57139208?rs 16939881 were not found associated with the stroke incidence.4. No gene-gene interaction was observed. Linkage disequilibrium was observed between rs3758269 and rs2989924.Conclusions AQP7 SNPs were associated with the risk of stroke in Chinese Han population. No association AQP9 SNPs and the risk of stroke were found.
