Objectives: Based on the study of medical cases of diabetes in Ming and Qing dynasties the paper tries to analyse and discuss the regularity of pathogenesis with syndrome, treatment and prescriptions of diabetes,.Methods: Medical cases of diabetes in Ming and Qing dynasties are retrieved, screened from the Chinese Medical Classic to obtain the ones fitting selection criteria. Then, the elements such as causes, syndromes, symptoms prescriptions and drugs of diabetes, etc. are extracted from the selected cases, and recorded into TCM inheritance software to establish medical cases database of diabetes. The data is analyzed through the statistic methods of frequency analysis, association rule analysis and so on.Results:1. Totally, 225 diabetes cases of medical works meet the selection criteria and are selected to research. 2. A total of 23 syndrome types are obtained, and its main syndrome types include Kidney Yin deficiency syndrome type, both Qi and Yin deficiency syndrome type, both Yin and Yang deficiency syndrome type, Lung and Stomach heat syndrome type, Lung heat and fluid injury syndrome type and so on. 3. About 11 kinds of diabetic causes are summarized. Exhaustion due to sexual indulgence dominates the first place, and other major causes include emotional disorder, improper diet, inappropriate medication after illness, pathogenic summer-heat and so on. 4. Position of pathological changes mainly lies in kidney, followed by spleen-stomach and lung-stomach. Several organs often are involved, with kidney, spleen-stomach and lung as major positions. 5. A total of 198 symptoms, and the first five are polydipsia, polyphagia, emaciation, polyuria, turbid urine. Combination law of these symptoms reflected the characteristic of diabetes, which is called “three more and one less”( polydipsia, polyphagia, polyuria and emaciation). 6. A total of 229 traditional Chinese medicines are sorted out. The topmost medications in frequency are medicines of nourishing yin and clearing heat, tonifying formula or expelling heat. 7. The associations between pathogenesis, syndrome, treatment and medication of each of five main syndrome types are briefly builded. The following are the associations of pathogenesis with syndrome, treatment and prescriptions of main syndrome types about diabetes in Ming and Qing dynasties: Kidney Yin deficiency with frenetic fire—Kidney Yin deficiency syndrome type—nourishing Yin and reinforcing kidney—Liuwei Dihuang Pills add and subtract; Yin deficiency with Qi consumption—both Qi and Yin deficiency syndrome type—clearing away heat and nourishing Yin—Qian Shi atractylodes Powder, Shengmai Powder, Sijunzi decoction add and subtract; Yin deficiency involving Yang—both Yin and Yang deficiency syndrome type—nourishing Yin and activating Yang with tonifying kidney and astringent—Jingui Shenqi Pill add and subtract; Lung and stomach's heating with consumption of body fluid—Lung and Stomach heat syndrome type—clearing away heat and nourishing Yin with relieving dryness—White tiger decoction, Coptis and tripe pills add and subtract; Lung heat with consumption of body fluid and Qi—Lung heat and fluid injury syndrome type—clearing heat and moistening Lung with inducing saliva and slaking thirst—White tiger plus ginseng decoction add and subtract.Conclusions: The general pathogenesis of diabetes in Ming and Qing dynasties belongs to the pattern of asthenia in origin and excess in superficiality. Asthenia in origin gives top priority to Yin-deficiency, involving Qi-deficiency and Yang-deficiency. Excess in superficiality gives top priority to dryness-heat, sometimes accompanying turbid dampness, stasis blocking or other pathogenic factors. The main syndrome types are Kidney Yin deficiency syndrome type, both Qi and Yin deficiency syndrome type, both Yin and Yang deficiency syndrome type, Lung and Stomach heat syndrome type, Lung heat and fluid injury syndrome type. And the relevant medications give top priority to recipes of nourishing yin and clearing heat, tonifying formula or expelling heat.
