The Association Between Galectin-3 And Aldosterone In Patients With Chronic Heart Failure

Abstract/Summary:
Objectives: Chronic heart failure is due a variety of severe heart disease or large vascular disease to the final destination of the blood hemodynamic abnormalities for pulmonary congestion and/or systemic congestion. And it is a widespread attention of health problem in the world. As a new type of biological markers, galectin-3 has close relations to the development, diagnosis, treatment and prognosis of heart failure. So it can be used as an indicator to estimate the severity and prognosis of heart failure. This study aimed to observe the association between plasma Galectin-3 and aldosterone level and cardiac structure or function in patients with Chronic heart failure. The results in order to provide the new information to further explore the physiology and pathology of heart failure and detection measuers of heart failure.Methods: In this randomized controlled study, a total of 100 consecutive subjects were recruited from the Department of Cardiology or Management Centre of Changsha Central Hospital from November 2014 to February 2015, including seventy-fir CHF patients and twenty-nine healthy subjects. Their gender, age, height, weight, and other general information are match. All CHF patients with New York Heart Association(NYHA) functional classification of Class II group(n=14), Class III group(n=32), Class IV group(n=25) were included. In addition, All CHF patients, according to left ventricular ejection fraction, were included: LVEF>40% group(n=36) and LVEF≤40% group(n=35). All patients’ serum Galectin-3 levels was measured by enzyme-linked immunosorbent assay(ELISA), aldosterone levels was measured by chemiluminescent immunoassay, BNP, liver function, renal function, blood lipids and other biochemical indicators were detected and cardiac function was evaluated by cardiac color Doppler ultrasound. All measure value was analyzed by using the 19.0 SPSS software.Results:1. Serum Galectin-3 levels(5.28±3.48), Ald levels(210.39±110.45) and BNP(1601.71±1328.38)were significantly higher in patients with HF(n=71) than that in control group(0.34±0.03、127.83±40.08、37.65±10.23, P<0.001).2. The serum Galectin-3 and BNP(1.64±0.74, 444.02±103.46) levels in HF patients with NYHA Class II were higher than control group(0.34±0.03, 37.65±10.23, all P<0.01), and Ald(97.86±19.59) levels lower than the control group(127.83 ± 40.08, P<0.01), the differences were significant statistically significant. Significant increases of Galectin-3(4.20±1.28, 8.70±3.89), Ald(177.09±49.70, 316.03±108.14) and BNP(1058.59±389.77, 2945.20±1374.55) levels were detected in HF patients with NYHA Class III-IV compared with those of the control group(0.34±0.03, 127.83±40.08, 37.65±10.23, all P<0.01). The levels of Galectin-3, Ald and BNP among those various subgroups have significant differences respectively(all P<0.01).3. Patients with LVEF>40%(n=36) had higher Galectin-3(4.37±3.60), Ald(177.66±105.11), BNP(1319.39±1394.89) levels than the controls(0.34±0.03, 127.83±40.08, 37.65±10.23, all P<0.05). Serum Galectin-3 levels(6.22±3.14), Ald levels(244.06±106.97) and BNP levels(1892.09±1207.82) were higher in patients with LVEF≤40%(n=35) than in patients with LVEF>40% and the control group(all P<0.05).4. The results form the correlation analysis showed that serum Galectin-3 in HF patients(n=71) was positively related to Ald, BNP, LVD and NYHA class(r= 0.976, 0.997, 0.399, 0.850, P<0.001, <0.001, =0.001, <0.001 respectively), But inversely related to LVEF(r=-0.366, P=0.002). Ald was positively related to BNP, LVD and NYHA class(r= 0.977, 0.381, 0.821, P<0.001, =0.001, <0.001 respectively), But inversely related to LVEF(r=-0.354,P=0.002).5. Receiver-operator characteristic curve(ROC) for diagnosis of HF showed the area under the curve(AUC) of Galectin-3 in HF patients(n=71) was 0.86, the best cutoff value was 2.39ng/m L and the corresponding sensitivity and specificity were 78.60%, 13.30% respectively. While the AUC of Ald was 0.71, the best cutoff value was 138.53pg/m L and the corresponding sensitivity and specificity were 71.40%, 33.30% respectively. The AUC of BNP was 0.97, the best cutoff value was 195.56ng/m L and the corresponding sensitivity and specificity were 98.60%, 3.30% respectively.Conclusion:1. Serum Galectin-3 level was significantly increased in patients with heart failure, correlating with the severity of heart failure.2. The level of Galectin-3 in HF patients was positively related to Ald and BNP. And it was positively related to LVD and negatively with left ventricular ejection fraction(LVEF). These results provide the new information to further explore the physiology and pathology of heart failure and detection measuers of heart failure.
